  UNE-P PAL order requests for ADD new service or Conversion As Specified requests DELETE “new service” or “as specified” conversions are placed using the following forms:

  Order processing for UNE-P PAL is manual, LSOG forms must be submitted to Qwest via facsimile at (888) 796-9089.

  Requests for multiple conversions to UNE-P PAL can be made on the same ADD local service request DELETE LSR, provided the request is based on service currently on the same Qwest CSR, for the same end-user, at the same location, and for the same due date.
