BBC INDEPENDENT PRODUCTION COMPANY

HEALTH AND SAFETY QUESTIONNAIRE


This questionnaire is to enable us to get an accurate and consistent assessment of an independent production company’s safety management. This is to enable us to work safely with third parties and to ensure we comply with UK legislation and best practice.
(Please note: Approval for your company lasts for a maximum period of 3 years. On occasion the BBC may require you to provide further information to provide assurance that appropriate safety systems are in place to undertake other activities which may constitute a higher risk or involve travel to a hostile environment.

Tips on filling in the questionnaire


  • A box has been provided for you to put your details in but please feel free to fill in the questionnaire whichever way you see fit. Whether that be point by point or as one long piece, it’s up to you. Just make sure you’ve covered all the points otherwise this can delay the process.

  • We do not vet productions that have already completed filming so please make sure you submit as early as possible. If in doubt contact your BBC BAM.

  • Please avoid writing “Please see Policy”. This takes us longer to process and may lead to us requiring clarification on various points.

  • Please try to give an accurate portrayal of what actually happens rather than what you think we want to hear. The clearer the information you provide on what you actually do, the easier it is for us to process.

  • If you have any questions about the form or process please feel free to contact us (details below). We don’t bite and we want to help get you vetted as quickly and easily as possible so that you can concentrate on what you do best, make programmes.




Please ensure you have covered information on all the points listed below:




  1. Company name, address, contact details and the number of staff you employ.

  2. On what date do you finish filming/recording? (NB Not delivery date)

  3. Who is ultimately responsible for the safety of others within the company? (This would normally be a board director or the MD but may be someone with a recognised health and safety qualification in larger companies.)

  4. How is safety responsibility delegated throughout your company?

  5. Who is responsible for completing risk assessments and ensuring that they are suitable and sufficient?

  6. Who do you go to for safety advice when something is outside your experience or knowledge? (Please include their contact details, training if appropriate and the nature of the relationship ie retainer, per production?)

  7. How do you stay up to date with legal changes and best industry practice?

  8. How are people told about hazards they are going to be facing at work and how they are controlled?

  9. How do you check that the people you employ or contract are trained, experienced and working safely?

  10. What safety training is required of/provided for staff?

  11. How do you check that staff/contractors are working safely on your project?

  12. What are your first aid/emergency arrangements during production?

  13. How are accidents reported on production?

  14. Please give details of any dealings you have had with any enforcing authority as the result of an accident or incident. (These include the HSE, Environmental Health Officers, Fire or Police forces in the UK or abroad).

  • A completed risk assessment from a previous production. (This does not have to be a BBC Production)

  • A copy of your company’s Health and Safety Policy (If Appropriate).

Personal information collected from this Questionnaire will be used to fulfil the BBC’s obligations under Health and Safety policy and legislation. It will be retained by the BBC for up to [6] years after the completion. It may be shared with other organisations, including our agents and contractors, with whom the risk or the control of risk is shared. You have the right to confirm that any information held about you is correct.
