MINUTES



  1. CALL TO ORDER AND ESTABLISHMENT OF QUORUM

The meeting was called to order at 11:19 a.m. in conference room W064 of the Indiana Professional Licensing Agency, 402 W. Washington Street, Indianapolis, IN 46204. A quorum was established.

A motion was made and seconded to adopt the agenda, as amended.

A motion was made and seconded to adopt the minutes from the December 7, 2009 meeting, as amended.

iV. PERSONAL APPEARANCES


  1. Korine Kakasuleff did personally appear before the Board to answer questions regarding her

positive response application for certification as massage therapist by grandfathering. A motion was made and seconded to issue the certification on probation. Quarterly documentation of attendance of NA meetings, criminal probation officer must submit reports. She would be eligible to petition for withdrawal of probation after she has shown proof to the Board that she has successfully completed criminal probation.

  1. Jessica Gullett did personally appear before the Board to answer questions regarding her

positive response application for certification as a massage therapist by grandfathering. A motion was made and seconded issue the certification on probation. She would be eligible to request termination of probation after she has shown proof to the Board that she has successfully completed her criminal probation.

A. INITIAL APPLICATION REVIEW
Andrea Stevens did personally appear before the Board regarding her application for certification as a massage therapist by examination. A motion was made and seconded to approve the application and issue her a certificate to practice massage therapy based on her license to practice as a massage therapist in the State of Iowa, the successful completion of the National Examination and that she has sufficient educational background in massage therapy.

VII. DISCUSSION
A. FSMTB Update
Barb Lis gave an update to the Board regarding the FSMTB.
B. Meeting Schedule for 2010 and beyond
The Board decided to cancel the April 5, 2010 meeting and is going to a quarterly meeting schedule for the remainder of 2010. The meeting scheduled for the rest of this year will be on: June 14, September 13 and December 6, 2010.
C. Alexandria School of Scientific Therapeutics, Inc.
The board received a complaint from a former student of Alexandria School of Scientific Therapeutics, Inc. which indicated that she did not receive enough training to pass a portion of the national examination. The Board advised that the student either submit a formal complaint to ICOPE or contact the school for additional assistance
E. Jennifer Messer

A motion was made and seconded to issue an Order to Show Cause to Jennifer Messer due to her unsuccessful completion of criminal probation.
