Form 6 M ENTOR G UIDELINES As anew mentor, you maybe wondering whereto begin your new mentoring relationship and what to include on the Action Plan (form 4). Although the Action Plan is to be largely dictated by the results of the Assessment Summary Form 3), there area number of other foundational issues that every mentor needs to address right from the start. These foundational issues are listed below and we encourage you to implement them into the Action Plan of the individual you are mentoring.
