- Admission and Discharge Card,
- Admission and Discharging Register
- IPD Morbidity and Mortality Tally
- PICHT tally
- Report forms ( Disease and IPD related section of service report form) Describe the benefits of data collection in these HMIS instruments Describe what and how to transfer data from these data collection instruments used in IPD to HMIS reports
