Policy/Procedure

The 340B drug pricing program provides access to reduced price prescription drugs to patients of a Federally Qualified Health Centers and a 340B contracted pharmacy.

NHC is required to recertify eligibility in the program annually.

Eligibility

A patient is eligible for the 340B program as long as they are an established patient at one of our site locations in either our Medical, Dental or Behavior Health departments. The patient must receive care from a Provider employed at the site or a Provider that is under contract or other arrangements.



The individual will not be considered an eligible patient if the only health care service received by the individual is the dispensing of a drug or drugs being requested.

The participating pharmacy (See appendix A) will not run any patients that have active state funded insurance plans through the 340B pricing to avoid any duplicated discounts.



Eligible Drugs

The 340B program covers the following outpatient drugs:



  1. FDA-approved prescription drugs;

  2. Over-the-counter (OTC) drugs written on a prescription;

  3. Biological products that can be dispensed only by a prescription (other than vaccines); or

  4. FDA-approved insulin.

Storage and Monitoring of Medications

  1. Medications may be purchased for clinical use through the 340B program.

    1. These medications unless injectable are not billable to a patient. As such oral medications will be prescribed to the patient to obtain on their own at a pharmacy.

    2. Signed approval from the Medical or Clinical Director is required for all non-dispensing medications; which is then sent to the participating pharmacy for ordering. Patients receiving injectable medications through clinic stock will be billed for this service.

  2. The medical staff will be responsible to keep an inventory of medications on hand.

  3. Medications due to expire will be destroyed according to our Medication Destruction Procedure.

  4. Providers should make any pharmaceutical requests to the medical staff who will order from a participating pharmacy. A list of participating pharmacies is listed in Appendix A. Most requests can be fulfilled overnight.

Procedure for Prescribing Medications

  1. Provider profiles are updated with the participating pharmacy (see Appendix A). The Director of Operations will notify the participating pharmacy at any time a Provider is no longer employed through Northern Health Centers, Inc. or begins employment. The participating pharmacy will update the provider profile to reflect accuracy.

  2. Providers can submit prescriptions under the 340B program through any of the following measures: electronic, phone, written.

  3. Some over the counter medications are available at a 340B discount. The Provider would need to follow the same procedure for prescribed medications using either electronic, phone or written measures.

  4. Providers will inform the patient of participating pharmacies in the 340B program. (See Appendix A)
