Examples of Major Hazards are condensate sea states collisions/allisions.
Examples of major incidents are major fire or explosion resulting in more than three fatalities crude oil spillage resulting in pollution of a large part of a river estuary and requiring extensive cleanup and remediation measures collision of helicopter, ship, road or rail tanker with the installation, resulting in a major incident.
Where the HEMP identifies Major Hazards, bow-tie models
(see Figure 5) are required to be developed to identify the potential Major Hazard release, escalation and consequence scenarios identify the controls, i.e. barriers and escalation factor controls, required to effectively manage these hazards to be tolerable and reduced to ALARP. The controls relevant to this manual are o the SCEs o an outline of the SCE’s performance requirements o an outline of the HSE critical tasks (i.e. the performance assurance tasks).
