Target Audience Physicians Nurses Nurse Practitioners Other Providers Other Health Care Professionals Social Workers Allied Health Professionals Pharmacists Care Managers Nursing Home Administrators Learning Objectives Discuss the scope of the problems related to re-hospitalization and unnecessary re-admissions. Outline the goals of the INTERACT nursing home care program. Describe the tools used in the INTERACT nursing home care program. Demonstrate how to complete the Quality Improvement Tool. Identify strategies for efficient, effective, and sustainable implementation of the INTERACT nursing home care program. Accreditation Physicians: The Dartmouth-Hitchcock Medical Center is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians. The Dartmouth-Hitchcock Medical Center designates this live activity for a maximum of 5.25 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity. Nurses: Dartmouth-Hitchcock Medical Center’s Nursing Continuing Education Council is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation. This educational activity carries 5.25 contact hours. Social Workers: This program has been approved by New Hampshire NASW for X (xx) Category I Continuing Education Units (CEUs). (NH NASW Authorization #xxxx). Faculty Amy Boutwell, MD, MPP is the co-founder of the STAAR (State Action on Avoidable Rehospitalizations) Initiative of the Institute for Healthcare Improvement (IHI). Since 2008, Dr Boutwell has been deeply immersed in the clinical, operational, policy, payment and political aspects of approaches to reduce avoidable rehospitalizations and improve care transitions. In June 2011, Dr Boutwell founded Collaborative Healthcare Strategies to support providers and communities in responding to the healthcare delivery improvement vision outlined in the Affordable Care Act. Dr Boutwell trained in internal medicine-primary care at Massachusetts General Hospital and is a practicing general internist, attends on the medicine teaching service at Massachusetts General Hospital and Newton-Wellesley Hospital. She is also an Instructor in Medicine at Harvard Medical School. Laurie Herndon, MSN, GNP, ANP is a Gerontological Nurse Practitioner at the Fallon Clinic and Worcester, MA, the Director of Clinical Quality at Massachusetts Senior Care Foundation, and the Senior Project Coordinator with the INTERACT Project. Since 2006, Ms. Herndon has been a Clinical Instructor in the Gerontological Nurse Practioner Program at the University of Massachusetts School of Nursing. She has worked as a project consultant for a number of New England Quality Improvement Organizations on topics including depression, falls, and medication management. Joseph G. Ouslander, MD is is Professor and Associate Dean for Geriatric Programs at the Charles E. Schmidt College of Medicine, and Professor at the Christine E. Lynn College of Nursing of Florida Atlantic University in Boca Raton Florida. Dr. Ouslander served as a Professor at UCLA for 15 years developing research, educational, and clinical programs. From 1996-2007 he served as the Director of Geriatric Medicine at Emory University. He is a past-President and Board Chair of the American Geriatrics Society, and serves as the Executive Editor of the society's Journal. He is a co-author of Essentials of Clinical Geriatrics and Medical Care in the Nursing Home, and an editor of Hazzard's Geriatric Medicine and Gerontology. Planning Committee Stephen J.
