70-year old man with recurrence of major depressive episode on escitalopram 10mg/day for 6 months with improvement then developed prominent loss of drive and motivation lasting 2 months without worsening of depression, obtaining score of 62 on Apathy Evaluation Scale. Agomelatine 25 mg/day added with good improvement and resolution of apathy upon complete switchover to agomelatine.
