Type 2: Permit’s Scenario
By using the old permit in Appendix, A (Class Ab bor Class B)

The following questions all from Old permit

1-Application and description box
1- work order title
The type of work either mechanical, electrical or Civil
2- Permit Nob The permit numbers
3- Location
The location in general, such as Lekwaire or Fuhood etc.
4- Work site
The specific location such as in Lekwaire Malan, B station etc.
5- No. in work party
The number of workers need for the work
6- Work description
The description for the work and all activities
7- Activity No. Refer to PTW system
The all-activity number included in the work (you can get it from the appendix of the SP book)
8- Area classification
The aera type either process facility, hydrocarbon or Non hydrocarbon
9- Associate permit number
If you have old permit and it is the same as your new permit, you should write the number of old permit
10- Work Order / EWP Nob It is a number come from the PDO planner engineer, he sends it to the company
11- Planner agrees in frozen plan
The permit in frozen plan or not
12- Hazardous area classification
Choose the zones type
