If you are employed by an L.E.A. or a Foundation School in Wales and you:-



  1. are familiar with the National Standards for Headteachers in Wales

  2. have read the “Guide for Applicants” section

  3. have undertaken the Personal Reflection exercise

  4. are confident you should proceed with this application

then please complete this form, remembering to sign and date it on page 13. Complete the referee details on page 13 (this should be your current Headteacher or Line manager). Do not make any entries beyond page 13. Print the form, show it to your chosen referee and ask him/her to fill in the details on pages 14 & 15. Finally, send it to the Wales NPQH Centre by 21st October 2005 at the latest.

If you intend financing your own training and you:-



  1. are familiar with the National Standards for Headteachers in Wales

  2. have read the “Guide for Applicants” section

  3. have undertaken the Personal Reflection exercise

  4. are confident you should proceed with this application

then please complete this form, remembering to sign and date it on page 13. Complete the referee details on page 13 (this should be your current Headteacher or Line manager). Do not make any entries beyond page 13. Print the form, show it to your chosen referee and ask him/her to fill in the details on pages 14 & 15 Finally, send it to the Wales NPQH Centre by 21st October 2005 at the latest.
