CERTIFICATION CREDIT

CRC/CVE/PVE Credit- Pre-approved forms available for all attendees.
LPC Credit: Licensed Professional Counselors may use the CRC Certificate of Attendance form to satisfy LPC requirements. The CRC Form can be duplicated and used to meet both needs.
CDMS, CCM, AND NCC Credit:
A Verification of Attendance section has been provided for your use in this conference program. Make sure that you have this form signed to have your attendance verified. This entire program, along with the signed section and the Post-approval Certification form will be sent to CCDMS, and CCCM when you are re-certifying.
