Conclusion: Membership to the Buhl Community Center provides access to exercise equipment, group fitness classes, and a method of attendance documentation for WW clients. To make this an approved Health Coaching + Fitness Program, WW will provide up to four sessions with a personal trainer. The initial and last session with the personal trainer will include the mandatory fitness assessments to document health changes. The WW health coach will be in contact with clients throughout the duration of the program to offer ongoing support calls.

Pennsylvania WISEWOMAN Health Coaching + Nutrition

Example of an Established Program

While it is ideal that the WW client participates in a Health Coaching + Wellness program to receive the most comprehensive health education, not all community establishments have the necessary infrastructure to provide the combination of physical activity and nutrition education. For this reason, this proposal includes the implementation of Health Coaching + Nutrition programs at facilities which are only able to offer nutritional education.

The following is an example of a Health Coaching + Nutrition program that WW clients would be approved to participate in. In the case that the selected Health Coaching + Nutrition program does not meet all the requirements to be recognized by WW for approval, WW is able to fill in the gaps by supplementing additional health education resources paid for by the program’s funds.

Cooking Matters” Health Coaching + Nutrition Program Proposal



Statement of Purpose: To provide WW clients with an additional nutrition education resource in their surrounding community. “Cooking Matters” programs will educate the client on what a balanced diet consists of, how to buy healthy foods on a budget, and food preparation/meal planning skills.

Benefits of Partnership: “Cooking Matters” provides WW clients with the tools needed to sustain a healthy diet while shopping on a budget. The educational classes and tours take place in the client’s local grocery store, offering a convenient location which removes barriers for our clients to attend.

Goals of “Cooking Matters” Health Coaching + Nutrition Program

  • Teach participants how to shop for a healthy diet while constrained to a budget.

  • Educate participants on balanced diet guidelines as outlined by “My Plate” materials, encouraging whole food options and denouncing use of any supplements, powders, or diet pills).

  • Teach participants meal planning/preparation.

  • Provide participants with healthy recipe ideas.

Goals of WISEWOMAN Supplement

  • Provide WW client with one-on-one sessions with a Registered Dietician who will offer an initial diet assessment, develop a personalized weekly meal plan for clients, and offer on-going support/motivation.

  • Provide health coaching calls to the WW client to ensure that any questions/concerns are addressed and to gain feedback on participation in “Cooking Matters” program.

  • Document client attendance in “Cooking Matters” program.

Cooking Matters” HC+ Program Description

  • Clients must be eligible for Health Coaching (at least one abnormal value) and demonstrate to the Health Coach a willingness to participate in the “Cooking Matters” Health Coaching + Nutrition program.

  • Client will be referred to local grocery store which hosts “Cooking Matters” courses and tours and will be paired with a Registered Dietician.

  • Registered Dietician will meet with client one-on-one to provide an initial diet assessment and to develop a weekly meal plan. WW will pay for up to 4, 30-45 minute individual sessions with the R.D. throughout the program.

  • Client will attend the hands-on cooking course program offered by “Cooking Matters” at their local grocery store.

    • Cooking courses are two hours long, offered once a week for six weeks.

    • Classes cover meal prep, grocery shopping, food budgeting, and nutrition.

  • Client will attend one individual grocery store tour with “Cooking Matters” leader.

    • Tours will focus on how to stretch budgets to purchase healthy food options.

    • Clients will be challenged to buy a healthy meal for $10 upon completion of tour. Clients are able to keep the food purchased in this challenge and are provided with educational materials to sustain healthy dieting such as recipe books, cooking utensils, and nutritional pamphlets.

  • WW health coach will conduct four follow-up calls with client to address barriers/concerns and offer on-going support to client in an effort to sustain participation in program.

    • One to be delivered at beginning and end of program, the other two to be spaced evenly throughout program duration.

Cost

  • Cooking Matters program cost : Free

  • One-on-one sessions with Registered Dietician : Up to 4, 30-45 min. sessions at $35 each = $140

  • Health Coaching calls : Four for $25 each = $100

Example of an Established Program

The Health Coaching + Community Linkages framework offers a different framework which allows clients to utilize the many resources available to them in their surrounding community. In this program structure, the WW client is referred to become a member of her local community organization. These organization sponsor many wellness initiatives which take place at nearby community sites (libraries, recreational centers, parks, etc.). WW will pay for the client to attend any class deemed appropriate for offering physical activity/nutritional education. WW health coaches will administer follow-up calls throughout the duration of the program to offer on-going support and address any questions/concerns the client may have.



The following is an example of the type of program a WW client may be referred to through her community organization. This example focuses on physical activity, however, referrals may be to classes focusing on nutritional education, or ideally, both physical activity and nutrition.

First Strides Running Workshop Health Coaching + Community Linkages Program Proposal

Statement of Purpose: To provide WW clients with an additional physical activity resource in their surrounding community

Benefits of Partnership: First Strides Running Workshop is a beginners walking and running club for women offered in Allentown, Bethlehem Township, and Hanover Township. First Strides will connect the WW client with a support system of community members who are motivated to exercise, thereby promoting program adherence. The workshop is held at local parks which offers a convenient location that reduces barriers for participation. The ten-week program structure takes a gradual approach for progressing from walking to jogging and is easily tailorable to suit every individual’s fitness level.

Goals of First Strides Running Workshop Health Coaching + Community Linkages Program

  • Educate participants on running techniques, stretching basics, and injury prevention.

  • Host weekly group runs/stretching sessions where community members support each other.

  • Encourage each participant to progress from walking to jogging/running at a pace suitable for their own fitness level.

Goals of WISEWOMAN Supplement

  • Offer on-going health coaching calls to WW client to answer questions/concerns and gauge participation in program.

First Strides Running Workshop HC+ Program Description

  • Clients must be eligible for Health Coaching (at least one abnormal value) and demonstrate to the Health Coach a willingness to participate in the Health Coaching + Community Linkages program.

  • Client will be referred to her local community organization.

  • Community organization will inform client about First Strides Running Workshop. If willing to attend, WW will pay for the program fee.

  • WW client participates in the 10-week running program where she participates in group runs and receives education on physical activity.

  • WW health coach contacts the client through health coaching phone sessions. One session must occur at the beginning and end of the program to collect feedback. Monthly phone calls are to be delivered throughout the duration of the program to gauge client’s participation and offer on-going support.

  • First Strides Running Workshop Program Fee: $50

  • Health Coaching Sessions : 4 at $25 each = $100

Evaluation of HC + Community Linkages Program Criteria

Due to the high level of variability among all the potential community wellness initiatives a client may participate in, each program requires additional approval to be considered a Health Coaching + Community Linkages program. A short summary of the program details and how the program would improve the WW clients’ health knowledge/participation in physical activity and/or healthier dieting is to be submitted for approval. Programs which are approved will be implemented as soon as possible.



First Strides Running Workshop would be approved as a Health Coaching + Community Linkages program as it encourages the client to increase physical activity through the group runs the program holds weekly. WW will supplement the program offerings by administering health coaching calls to offer on-going support to clients. The combination of these two initiatives serve to motivate a client to adopt healthier lifestyle behaviors, therefore, this program would be approved for client use.

2016 Health Coaching PLUS Pilot Evaluation Plan
Purpose: To evaluate the Health Coaching PLUS pilot initiative for the PA WISEWOMAN program.
Primary Goals:

  1. Determine if the Health Coaching PLUS pilot program is administratively feasible

  2. Determine WW client health outcomes after participating in the Health Coaching PLUS pilot program.

  3. Utilize feedback in order to identify necessary areas of improvement in the Health Coaching PLUS pilot program for both provider and client.


Indicators:

  1. Ease of implementation

    1. List of barriers found during implementation

      1. Survey program providers and screening contractor to identify issues.

        1. How easy was implementation?

        2. What problems (if any) arose?

        3. Was there additional administrative burden placed on other program areas as a result of implementation?

        4. What changes could be made to simplify the implementation process?

  2. WW clients’ health outcomes after participating in the Health Coaching PLUS Pilot program:

    1. Data collection

      1. For HC+ Fitness and HC+ Wellness programs, MDE data from screening and post-assessment will be used. If the program offers an assessment of client outcomes/participation, the program should provide the assessment data to WW client. The client should then relay the assessment data to WW health coach for program evaluation.

      2. For HC+ Nutrition programs, if the program offers an assessment of client outcomes/participation, the program should provide the assessment data to WW client. The client should then relay the assessment data to WW health coach for program evaluation. Pre- and post-health assessments delivered by screening contractor staff will measure client anthropometrics and behaviors.

For HC+ Community Linkages programs, evaluation will be more general due to variability among programs which the client may participate in. If the program offers an assessment of client outcomes/participation, the program should provide the assessment data to WW client. The client should then relay the assessment data to WW health coach for program evaluation. Pre- and post-health assessments delivered by screening contractor staff will measure client anthropometrics and behaviors.

      1. For all programs, tracking client participation will be the responsibility of the program leader. The program leader may document attendance however he/she deems most fit; however, monthly attendance logs should be submitted to WW if possible. Only in the case that the program leader is unable to track attendance, the WW client assumes responsibility for self-reporting her own attendance and providing a monthly log to WW.

        5. Do you consume the recommended servings of fruit each day? Vegetables? Whole-grains? Fish? Lean meat? Low-fat dairy?

        6. Have you cut down on drinking beverages with added sugars after completing the program?
