Email box

The patient registration email box should only be used when patients are unable to be connected with the Registration Department.

The email that should be sent should include:


  • Patient Name,

  • MRN or Date of Birth,

  • Reason for Transfer, and

  • Telephone number indicating best time to call during business hours.


Note: Include only one patient per email.



Direct phone number

While we strive to complete all registration updates at the time of scheduling, that is not always possible. Every effort should be made to get complete information at the time of scheduling, since that is when our patients are most committed to providing information we need. Patient’s with missing information at time of scheduling should be advised to contact Registration to provide us with the information.

Contact Registration Job Aid




Introduction

The Contact Registration job aid has been created for patients who are unprepared to complete a registration at the time of scheduling, and agree to contact the Registration Department according to the criteria on the form.
It can be used as follows:

If appointment is made…

then…

in person,

give completed job aid to patient.

Note: The appointment information is optional if you’ve already provided it in another format.

by phone,

read the top part of job aid, ask your patient to write down the information and follow-up with Registration.

by referring MD’s staff,

ask referring MD staff to read top part of job aid to the patient when notifying them of their scheduled appointment.

Note: Provide copies of job aid to referring MDs staff for their referred patients.

Financial Counseling

When a patient needs to apply for Health Safety Net (HSN, formerly Free Care) or MassHealth, the call should be transferred, or patient referred, to Financial Counseling.
