2017 Championships Diving Entry Form Saturday July 8th, 2017

Eligibility: Open to all GRPA, GHSA, USA Diving members or other interested individuals.

Rules: Governed by United States Diving.

Facilities: Two – 1 Meter & One – 3 Meter

_{Dives for Competition}

7 & Under - (3 dives) All divers shall perform a forward or back jump (100 or 200 A,B,C) – using a DD of 1.0. The jump shall be done at the beginning of the competition and shall be followed by two dives from any group, using true DD.

8 & 9 - (4 dives) All divers shall perform a forward and back jump (100 and 200 A,B,C) using a DD of 1.0. These jumps shall be done at the beginning of the competition and shall be followed by two dives in any group, using true DD.

1 0 & 1 1 - (4 dives) All divers shall perform a forward jump (100 A,B,C) or a back jump (200 A,B,C) using a DD of 1.0 at the beginning of the competition and shall be followed by three dives from at least two different groups, using true DD.

1 2 & 1 3 - (4 dives) All divers shall perform a forward dive 101 (A, B, or C) using a DD of 1.8 one/three meter, plus three dives from two different groups. Any dive on the table may be used. The optional dives shall be calculated with their true DD.

1 4 & 1 5 a n d 1 6 & 1 8 - (5 dives) All divers shall perform five dives from at least three different groups, with no more than two dives being from any group. Dives shall be calculated with their true DD.

** 3-meter list will be the same as one 1M in all age groups.

^{Except for the jumps will be line-ups (001/002} A,B,C^{) – using a DD of 1.0.}

^{Line-ups are judged how you enter (only) the water.}

NOTE: Jumps will be allowed if not already required. Example - If you are doing a front jump in 7 & U, you can do a back jump as one of your two dives.

Stay on Tara Boulevard for 5.4 miles and turn right at Deputy Rick Daly Memorial Boulevard. Follow the signs to the Jim Huie Recreation Center. (The recreation center will be on your left)

      Participating or assisting other in participating in the Championships may involve RISK OF INJURY TO ME, INCLUDING DEATH, LOSS OR DAMAGE TO ME OR MY PROPERTY, or other consequences, which might result not only from my own actions, in actions or negligence but also the actions, in actions or negligence of others, the rules play, or the conditions of the premises or of any equipment used;

      There may be OTHER RISKS not known or not reasonably foreseeable; and Understanding All of the Above,

      I ASSUME ALL OF THE ABOVE RISKS AND RELEASE, WAIVE, DISCHARGE, HOLD HARMLESS, INDEMNIFY AND COVENANT NOT TO SUE:

      The State of Georgia or any of its agencies, District Sports Festivals, the Georgia State Games Commission, the Georgia Sports Foundations, its Commissioners, its Board of Directors, its employees, agents, volunteers, coaches, trainers, or officials affiliated with their programs;

      Any affiliated subsidiary, successor, organization, or related companies or businesses, other participants, participating or sponsoring municipalities, governmental agencies, international organizations, agencies, sponsors, or advertisers, the respective administrators, officers, directors, agents, representatives, employees, or volunteers of such entities or organizations;

      The National Congress of State Games (NCSG), the United States Olympic Committee (USOC) and/or their respective representatives, officers, directors, employees, agents, successors and assigns;

      Owners, lessors and lessees of premises used to conduct the Games FROM ANY AND ALL LIABILITY FOR INJURY, INCLUDING DEATH, LOSS OR DAMAGE TO PERSON OR PROPERTY, OR ANY OTHER CONSEQUENCE in connection with entry in or arising out of participation in, performance in or lack of performance in, including travel en route to and from the Championships.

      Prior to participating as an athlete, I, or in the case of a minor, a parent or guardian, will INSPECT the facilities and equipment to be used, and If I believe same to be unsafe, I will immediately REPORT such condition(s) to the athletic coach, supervisor or official connected with the Championships of same and either DECLINE TO PARTICIPATE or ASSUME THE RISK of participating;
      
   2. 
      I will ALLOW my PHOTOGRAPH, PICTURE or LIKENESS and/or VOICE to APPEAR in any official documentary, promotional (including and all advertisements), television, radio or film coverage of the Championships, WITHOUT COMPENSATION.

      I CONSENT TO:

      ALL EMERGENCY MEDICAL TREATMENT as may be deemed appropriate under existing circumstances by medical personnel or personnel associated with the Championships. I HAVE READ THIS FORM IN ITS ENTIRITY AND HAVE PROVIDED TRUTHFUL INFORMATION.
