Affiliate Program Intake Questionnaire
The following is the affiliate program questionnaire that you need to fill out and return to YOUR COMPANY before work begins. Your answers will guide us in doing the best job possible for you. Your information will be kept strictly confidential.
If we speak on the phone, your call will be recorded for future reference. Calls must be pre-scheduled and are included as billable time. It’s typically a good idea to do a weekly call check-in. This would normally be 15-30 minutes in length once the program is running.

It will also help you plan and focus on what you want to create with your business. Answer as many questions as you can and be as clear (not long winded) as possible. Who is your competition? It helps us to know your major competitors. If they have affiliate programs, we will sign up for them. Please list 3-5 major competitors and their web sites

List questions you commonly get about your product. Anything that you are often asked? This will help us when writing copy for your promotions.
Step 2 – Describe Your Target Market:
Who is your ideal customer or client, your target market?
Step 3 – Describe Your Perfect Affiliate:
Who is your idea affiliate? Is it people who have membership sites, those who sell similar products, customers only, etc – describe your idea affiliates.
Step 4 – Your Business:
We ask you supply any materials you can about your business such as media kits, press releases, product descriptions, links to sales pages, etc. This helps us get a bigger picture of your product and business so we come look at it from the perspective of a customer. We want to know how your customers see and feel about your product.

Step 5 – Order Process & Affiliate System:
Walk us through an order of your product, including every page that a customer will hit on as they go through.

Describe any testing and/or tracking you have done on your salespages.

What is your conversion rate? List any and all conversion rates you know of. For example, your email open rates, sales conversions, etc.

Describe your current affiliate program setup. Walk us through this with everything you currently have set up. Do you have a customized affiliates area, special commissions set up, affiliate autoresponders, instructional materials, etc?

Do you currently have any promotional materials for affiliates? Where are they located?

How many affiliates do you have signed up currently? Where did they come from? Describe all known sources?
Step 6 – Potential Affiliates Brainstorming:
The absolute best people to contact about your affiliate program are people who already know like and trust you. As part of our service we will help you recruit new affiliates, but we do not believe in randomly contacting website owners who’ve never heard of you. That’s NOT the best use of our time and your money. We will help you come up with contact plans and actions for those who are already in your circle of influence. Once we have exhausted your contact list, we will create plans to expand that list by strategically targeting new businesses and potential affiliates. With that being said, we need to know who you know to get started.

Who do you know right now who would make a good affiliate for your program? List everyone you can think of, what their business is, why they would make a good affiliate and how you know them. This will help us create a plan of contact for each person that best suits THEIR needs.

Who are your best customers? List a brief description of your best customers. What type of business do they own and what is the advantage to them for becoming an affiliate.

Conferences – Think of live events or conferences where your potential affiliates would spend time. List any events where you would like to network and spread the word about your affiliate program. Do you have any lists of people you’ve already met (stacks of business cards) that can be contacted as potential affiliates? List their names and websites.
