Please use the following performance scale to evaluate student’s performance at the mid-semester mark.

PC= Pre Competence. The student does not or rarely demonstrate an ability to integrate the values, knowledge and skills associated with this competency; and needs constant supervision, reminders and extra time to complete the task or activity.
EC= Emerging Competence. The student is beginning to demonstrate or integrate the values, knowledge and skill associated with the competency with less reminders or prompts.
C= Competence. The student demonstrates or integrates the values, knowledge and skills associated with this competence with consistency and with independence. He/she uses supervision for consultation appropriately.
NOPC= No Opportunities to Practice Competency: The student has not had an opportunity to practice this competency at our agency.

In your professional judgment, is the student’s performance: satisfactory, or unsatisfactory? Please make any other comments that would be helpful for the student in guiding their work to be accomplished during the remainder of this field practicum experience.
Satisfactory: The Student’s skills are applied quite consistently, with some gaps; the student is on task towards developing his/her core competencies as expected for a graduate level social work intern.
Unsatisfactory: The student has had the opportunity, but has not developed any of the core competencies even at the beginning phase of skill development. The student is in jeopardy of receiving a recommended final grade, less than a “B. I would like to meet with the Field Liaison.
