North Atlantic Conference SOCCER REFEREE GAME-DAY SPORTSMANSHIP REPORT This form must be completed by the head referee immediately upon completion of the contest in order to assist the NAC in the improvement of sporting behavior for the good of the sport of soccer and development of our student-athletes. Failure to complete this form may result in delay of payment. Contest Date:______________ Site:_______________________ Competing Teams:_________________________________ If one or more yellow and/or red card(s) was issued during today’s contest please indicate the type of infraction using the following codes: YC1 - Unsporting Behavior - Reckless foul RC1 - Serious foul play YC2 - Unsporting Behavior - Tactical foul RC2 - Violent Behavior YC3- Unsporting Behavior - Taunting, aggressive behavior, lack of respect for game or opponent RC3 - Fighting (NCAA protocol must be followed) YC4 - Unsporting Behavior - Simulating a foul, exaggerating an injury RC4 - Spits at an opponent or any other person YC5 - Persistent Infringement upon rules of the game RC5 - Denies team obvious goal scoring opportunity by handling YC6 - Dissent by word or action RC6 - Denies OGSO by any other means YC7 - Uses profane language in an incidental manner RC7 - Hostile or abusive language - threats, obscene behavior YC8 - Delays restart of play RC8 - Receives a second caution YC9 - Fails to respect required distance on a free kick * All ejections require supplemental report via email to assignor within 24 hrs YC10 - Leaves or enters the field without referee permission
