52- Explain the life cycle for the permit
1- PDO planner engineer will produce plan for two weeks (frozen plan, then he sends the plan to the contractor planner engineer
2- Contractor planner engineer will receive the plan with all activities
3- Contractor engineer will send the plan to the permit applicant
4- Permit applicant will apply the permit either class A or class B. If it is
class A then a. site visit by RS and PA b. Authorize the permit by RS c. Approved by production coordinator d. Sign other effected custodian if any And if it is Class B: a. Authorize the permit by RS b. Approved by production coordinator if there is full isolation c. Sign other effected custodian if any
5- PA will choose the PH
6- PH will accept the permit if he agrees with it
7- PH will chick the permit and all attachment
8- PH will chick all material and vehicle that he will use
9- PH will go to area authority to validate the permit
10- AA validate the permit
11- PH will go to the site and he will conduct TBT
12- PH start the work
13- PH end the work
14- PH will go to the AA to suspend the permit

53- What are the responsibilities of permit holder
1) To ensure the work content and the Requirements of Job HS Plan
2) To Ensure Permit is validated before starting the work Each day.
3) Displays Permit at the work site in a plastic wallet.
4) Ensure TRIC throughout the work and "Pause and Check" is carried out.
5) Stays in close visual contact with work site at all times to supervise the work party
6) Stops work and move all personnel to a safe location if work site conditions are unsafe
7) Suspends or cancels the permit.
8) Return the permit to the Area authority
9) Get validation from the area authority
10) Understand the content of the permit and Job HSE Plan.
11) Brief the content of Permit & Job HSE Plan to the crew.
12) Display the Permit
13) Check and inspect all the tools & equipment as per HSE Plan.
14) Conduct gas test (if required) Suspend the permit for any emergency Suspend the permit if the period expired.
15) Cancel the Permit on completion of the Job
