Develop clear referral pathways and effective referral systems.
Encourage doctors, nurses and support staff to develop greater awareness about disability inclusion processes and a stronger internal system protocol to ensure patients with blindness and low vision are always referred to services.
Develop a process to check the follow-up of patients. For example: within two weeks, within three months and once a year. Ensure patients understand why, what and when follow up is required.
Dissemination of this information to the patients should be in a clearly understood and timely manner.
Step 6: Eye Health Promotion Activities
Use a variety of community based eye health promotion activities.
For example: Basic eye health information using plain language with clear pictures. Use simple auditory information through radio, TV or announcements in public places.
Standardised collection of information regarding the presence of disability and types of disability. Data collection must be well coordinated among the different service levels (commune, district, and province).
This information should be in harmony with the Ministry of Health recommendations for disability data collection (if they exist) so that it can be integrated into the national health information system.
