samples of INTEGRATED FOLDER, SERVICE CARDS,
MASTER PATIENT INDEX CARD, TRACER CARD, SERVICE ID CARD and APPOINTMENT CARD. Allow a few minutes for the participants to review the instruments. After about 5 minutes, ask which instruments will be used at each step of the client/patient flow. Facilitate discussion using the flip chart with client flow diagram putting the names of the appropriate record-keeping instruments that are/will be used at each step of client/patient flow. Clarify that instead
of multiple patient folders, only one integrated individual folder will be used for an individual client/patient.
