Banner Message Request Form

To request a banner message be posted to the GHP Web Portal, complete the steps below per banner message:

  1. Save the form as follows: Banner Message Request - date of request

  2. Attached the completed form to the iTRACE action item. The iTRACE action item is assigned to Rolon Sullivan, Publications Coordinator.

NOTE: All completed banner message request received by noon will be posted to the Web Portal that day, and can be viewed on the Web portal the next day.
