Mid-Atlantic Ballet Camp 2015 - General Information
Dear Family,
We’re very pleased your child is registered for Mid-Atlantic Ballet Camp.
The dates are June 22-June 26 and July 27-July 31.
Driving Directions & Parking Information
Mid-Atlantic Ballet (MAB) is located at 104 East Main Street (behind the former IHOP), Newark, DE. To access the parking lot (#3), use the driveway next to Abbott’s Shoe Repair. Take a ticket from the kiosk. Drive straight down the aisle, make a right and go down two parking aisles. You will see our logo on the glass door at the back of the building. We are down the stairs.
Camp Hours, Drop-off & Pick-up
Ballet Camp hours are 9am - 4pm, Monday through Friday. Parents must sign campers "in" and "out" everyday. Please make a note on the sign-in/out sheet if someone other than yourself is authorized to pick-up your child from camp. Parents needing to bring a camper late, or pick them up early, should notify the camp faculty.
Daily Camp Schedule
The daily camp schedule for Ballet Camp includes up to three dance classes per day including ballet, jazz, stretch, and modern; related arts, crafts, and activities, lunch, quiet/rest time, snack time and play/free times. Most camp activities will take place inside the building. Weather permitting, lunch and play time are held outside in the park behind the building. Dance classes are structured around the abilities of the individual students.
Faculty and Supervision
MAB Artistic Director Patrick Korstange and Faculty Members Sandra Davis and Morgan Sweede will supervise the campers at all times. Our director is certified in First Aid and CPR.
Emergency Information
Parents needing to contact a camper in case of emergency should call (302) 266-6362. If a camper becomes seriously ill or an emergency arises, we will call 911 first. Every effort will be made to contact the parents via the phone numbers provided on the camper's registration form. Please make sure this information is completely filled out on your child’s registration form.
Lunches and Snacks
Ballet Campers need to bring their own lunch and beverages. Please make sure your camper's name is clearly printed on their lunch. A refrigerator and microwave will be available for supervised camper use. Please instruct your child to NOT share food with other campers.
What to Bring To Camp:
*To wear during dance classes: Continuing MAB students should be prepared to change into the dress code of black leotard, pink tights, pink wrap skirt, pink ballet slippers, and a with hair in a bun. Gentlemen will wear plain white fitted t-shirt, black leggings or straight leg sweatpants, and ballet shoes. The dress code is recommended for new students and campers; however, comfortable fitness clothing may be worn. Pink canvas or leather ballet shoes are required. Long hair should be secured off the face and neck into a ponytail or bun.
NO costumes please!
*To wear during lunch and outside play time: cool, comfortable clothing with socks and sneakers. Campers will be given a MAB Ballet Camp shirt on the first day of camp and they should try to wear it every day!
*Please label all personal items with the camper’s name. MAB cannot be responsible for lost items. Campers will be provided a locker to leave their property at MAB throughout the week.
*Water Bottle
*General health and medical record form, if not previously submitted.
*Lunch and beverages
*A small pillow and sweatshirt or small blanket for quiet time (MAB provides mats)
*Dance books, music, games and videos to share with other campers, if desired.
*Optional sunscreen, sunglasses, and insect repellant for picnic lunches (weather permitting).

Camper Evaluations and Registration for Classes at MAB
All campers not previously enrolled at MAB will receive an evaluation by the artistic director for placement in the fall semester of classes at MAB. Patrick will be available to speak with parents of prospective students on the last Friday of camp. At this time, parents will also be able to register their camper for the fall semester of classes which will begin Monday, September 3th. Many previous campers are now enrolled at MAB…some are now in the Mid-Atlantic Ballet Company!

Waiver & Release
I understand that Mid-Atlantic Ballet may from time to time take photographs and/or videotapes of the student enrolled on this form and I authorize the Mid-Atlantic Ballet to use these photographs and videotapes for archival and publicity purposes. Recognizing the risks of illness and injury inherent in any dance program, I am participating upon the express agreement and understand that I am hereby waiving and releasing Mid-Atlantic Ballet, it’s employees and board of directors from any and all claims, costs, liabilities expenses or judgments, including attorney’s fees and court costs arising out of my participation in Mid-Atlantic Ballet’s programs or any illness or injury resulting there from, and thereby agree to indemnify and hold harmless Mid-Atlantic Ballet, it’s employees, and board of directors from and against any and all claims except for illness or injury resulting from gross negligence or willful misconduct on the part of Mid-Atlantic Ballet. I hereby execute and deliver the Waiver and Release to induce Mid-Atlantic Ballet to permit me to participate in its programs.
