Community Health Planning 2013

A total of four regional meetings were held to present community health needs assessment data for prioritization. The community meetings were held on:



January 17, 2013-Manchester Civic Center

January 24, 2013-Kimball Medical Center, Lakewood

January 31, 2013-Southern Ocean Medical Center, Stafford

February 20, 2013-CHEMED, Lakewood.

A total of 97 stakeholders attended from 56 county agencies


Thirteen Health Issues were identified in all four meetings, which were narrowed down to:

Five Prioritized Health Issues for Ocean County:

1- Childhood Obesity

2-Behavioral Health: Mental Health & Substance Abuse

3- Immunization Compliance

4- Chronic Disease Prevention and Education

5-Access to Care


The Ocean County Community Health Improvement Plan for 2013-2017 with strategies for action will be released in January 2014 for agencies to review and implement. A promotion and publication of the plan includes website access, presentations to community and faith-based organizations and county agencies. An annual update of the plan will follow every year.

The Ocean County Community Health Improvement Plan written in December 2013 will be published and promoted with its strategies and action plan during 2014.




Revision of the Public Health Practice Standards- NJ DOH convened various “Technical Assistance Groups” to revise the Practice Standards for Local Health Departments which expired on August 2013. Each department had a designated group to review its contents. The Community Health Needs Assessment group reviewed completely the local health department’s requirements for Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP). Meetings were held on March 22, April 12, April 17, May 1, and May 29, 2013. CHNA/CHIP Standards will be following the Public Health Accreditation Standards.

            Local Health Departments are required under Public Health Practice Standards of Performance for Local Boards of Health in New Jersey, N.J.A.C. 8:52-5.3 Community Public Health Activities, to report the status of the community’s health and how it compares with corresponding objectives set forth in Healthy New Jersey 2020.  This includes objectives for the Community Health Improvement Plan.   The Ocean County Health Department will continue to examine how we incorporate the 2020 objectives into our varied programs at the Health Department and into the new Community Health Improvement Plan, with the goals and objectives to be determined in 2013.   

            Many objectives of Healthy People 2020 focus on areas that the OCHD, either by itself or in our many collaborative efforts will look further to improve and enhance.  Some of the new topics listed in Healthy People 2020 are Adolescent Health, Health-Care Associated Infections, Health-Related Quality of Life and Well-Being, Older Adults, Preparedness and Social Determinants of Health.  Healthy People 2020 objectives also include many of the chronic illnesses that we see today.  The “Live Healthy Ocean County Program”, which offers preventative screening and education, and the “Take Control of Your Health” program, which is an evidence-based program from Stanford University teaching those with chronic illnesses how to successfully manage their disease, are addressing many of the objectives of Healthy People 2020.   In addition, the Ocean County Health Department is proud of its continual work with the Ocean County Office of Senior Services.

            Over the years, the OCHD has worked collaboratively with our other County agencies, hospitals, Federally Qualified Health Centers, behavioral health agencies, schools, and many others to identify the best resources and referrals to appropriate agencies to fit the needs of our County residents. There are many areas of Healthy People 2020 that we do not address but we certainly have an implemented referral system to resources addressing those issues.

            The Health Advisory Committee, consisting of the two area health departments, the two Federally Qualified Health Centers and the four area acute care hospitals, began working on strategic health planning for Ocean County residents. 

            The 2012 Ocean County Community Health Needs Assessment Data is a compilation of primary data collected from the above agencies, as well as from various secondary data sources with the purpose to assess Ocean County community health needs.   The partnering of various healthcare agencies allows for sharing of data and the avoidance of duplication of services while focusing more on the health needs of the community. The partnering has offered the ability to provide up- to-date information directly from the agencies who are the primary data collectors.

           

            The goal of the Health Advisory Group is to examine the strategic direction of health care and delivery of services in Ocean County and to increase awareness and provide an annual report on healthcare and prevention programs in the county to state, county, local officials, and residents.

            Needs assessments will take place in the beginning of 2013 in different geographical areas of Ocean County.  Participants will be asked to identify five health issues that they envision as a priority in Ocean County. In choosing the five areas, we will be looking at the:


  • Morbidity and mortality severity associated with the health issue

  • The relationship of the health issue to other community issues such as economy, population, the overall health system

  • Existing resources already available to address the health issue

  • The feasibility for the county to address the specific problem

            The Health Advisory Group will serve as the force in strategic health planning for Ocean County residents by identifying and assessing the health needs in the county thru prioritizing, and identification of future strategies in improving healthcare in Ocean County thru grant writing opportunities or by improving collaborative services amongst healthcare and community and faith-based agencies.   The Community Health Improvement Plan is targeted for 2013 and we look forward to presenting this is our 2013 Annual Report.
