  1. Proposed methods to evaluate impact of health coaching – including what measures the grantee proposes to use that will add to the evidence base

Pre and post-fitness assessments will be conducted for each WISEWOMAN client. Suggested measurements include body weight, body composition (BMI), cardio fitness, strength fitness, blood pressure, waist to hip circumference, etc. The data provided by these assessments will document the effectiveness of the program in increasing the health outcomes of WW clients. This data can then be compared across differing programs to identify which frameworks prove to be the most successful. This information will add to the evidence base and will be of benefit for future implementation of additional lifestyle programs.

The evaluation staff will develop a process to collect a personal narrative from WW clients as well as any recommendations for changes or future options. The evaluation will also include utilizing clinical data to identify changes or improvements at the post assessment. This information will be critical for determining the effectiveness of the proposal herein and can be used to tailor future proposals to more accurately meet the needs/preferences of WW clients.

Data collected through the fitness assessments as part of the Health Coaching PLUS programs will provide qualitative measurements to determine the success of the proposal in achieving health outcomes.


  1. Proposed plan for follow-up when health coaching is complete (interim assessment of patient progress and reinforcement of goals) – how/when will program expect women to return to complete follow-up assessment

Upon completion of the Health Coaching PLUS program the WW client will complete a post-assessment at the screening clinic. This is to be conducted immediately after Health Coaching PLUS program is completed. The program will reach out to clients who participate to complete an evaluation.

Pennsylvania WISEWOMAN Health Coaching + Wellness

Examples of Established Programs

Enclosed are examples of current YMCA programs that have shown to improve the health of participants. The programs vary by their means of delivering education on physical activity and nutrition and have costs ranging from $50.00-$170.00. All programs have been supplemented with additional health resources funded by WISEWOMAN, making all five approved as Health Coaching + Wellness programs.

The purpose of this proposal is to grant community establishments the freedom to develop their own wellness programs that adhere to the guidelines outlined by WISEWOMAN. This increased flexibility in program structure will allow for the creation of more wellness programs which can be utilized by WISEWOMAN clients. The enclosed programs serve as examples of frameworks that have been successful for past participants. Community establishments may consider mirroring various components of these examples in the creation and implementation of their own program.

Note that not all programs listed meet the required guidelines of WISEWOMAN. Additional supplements have been funded by WISEWOMAN to fulfill requirements. Each program is detailed by an outline as well as an evaluation table indicating where WISEWOMAN supplements are necessary.



The following YMCA example Health Coaching + Wellness programs are described:

  • Nutrition Made Easy

  • Weigh to Go!

  • Choose 2 Lose

  • Jump Start

  • Certified Health Coach (Personal Training + Nutrition Education)


Nutrition Made Easy Health Coaching + Wellness Program Proposal

Statement of Purpose: The purpose is to pilot a new initiative for current WW participants to become an active member at the YMCA and participate in the Nutrition Made Easy program.

Benefits of Partnership: Nutrition Made Easy is a flexible program created by the YMCA consisting of 5 educational sessions which teach participants how to maintain a healthy diet through education on macro and micronutrients and an emphasis on “clean dieting”. WW clients will receive education on how to include and track the proper amount of fats, carbohydrates, protein, and fiber in their diet. The principles of reverse dieting will also be addressed, therefore, clients will learn how to control their own metabolic rate. Taught by a Certified Personal Trainer, these lessons will encourage WW participants to adopt healthier lifestyle choices to decrease their risk for cardiovascular disease. In addition to the Nutrition Made Easy program benefits, WW will provide a four-month YMCA membership so clients have access to exercise equipment, thereby promoting regular physical activity.

Goals of Nutrition Made Easy Program

  • Provide direct nutrition education through sessions with a certified wellness trainer.

  • Provide WW client with an on-going support network to encourage healthy lifestyle choices.

  • WW participants will adopt a healthier diet and experience weight loss



Goals of WISEWOMAN Supplements

  • Provide WW clients with a four-month membership to a local YMCA to encourage physical activity and healthier lifestyle choices.

  • Provide another community based health coaching option for WW clients to choose from.

  • Provide WW client with an on-going support network to encourage healthy lifestyle choices.

  • Establish proven effectiveness of WW partnership with YMCA Nutrition Made Easy program with the aim of implementing similar programs at additional YMCA locations.

  • Maintain WW clients’ active membership by providing them with connections to scholarship/reduced-fee membership rates upon conclusion of the Nutrition Made Easy program

Nutrition Made Easy HC+ Program Description

  • Clients must be eligible for Health Coaching (at least one abnormal value) and demonstrate to the Health Coach a willingness to participate in Nutrition Made Easy program.

  • Health Coach will conduct monthly check-ins with the client to discuss weight loss, YMCA attendance, and barriers/success in health improvements.

  • YMCA certified wellness trainers will serve as the health coaches for the Nutrition Made Easy program. Certified wellness trainers will offer the following services to WW clients

    • Macro and micronutrient education

    • Reverse Dieting Training

    • Dietary tracking lessons

    • Coaching and nutritionist support for developing practical behavior change strategies

    • Document WW client attendance in the Nutrition Made Easy program data system.

  • Clients will be given tracker sheet to record each day they attend the YMCA for physical activity purposes and the type/rigor of exercise performed.

    • YMCA certified personal trainer will review tracker sheet to ensure that what the client documents is accurate. This data will be essential to tracking the progress/effectiveness of the program.

  • YMCA will track WW clients’ attendance.

    • When a client enters the facility and scans membership card the date and time will be recorded.

    • Monthly YMCA attendance logs will be provided to WW program in order to have documentation of the days participants utilized membership.

  • Prior to completion of program, clients will be connected to resources of scholarship-based and reduced-fee memberships enabling them to continue membership after Nutrition Made Easy program is completed.

Conclusion: The Nutrition Made Easy program offered by the YMCA provides a clear example of how the nutrition component of the WW wellness program may be structured, however, this program lacks all physical activity/exercise education components. Several supplements must be added to the program in order to warrant it worthy of being recognized as a WW Health Coaching + Wellness program. First, the WW client must be provided with a 4-month membership to the YMCA at minimum. To fulfill the personal training requirement clients must also be offered up to 4, 30-45 minute sessions. The personal trainer must also complete both a pre- and a post-fitness assessment in order for health improvements to be measured. If these supplements are added to the Nutrition Made Easy framework, then this program will meet all requirements to be recognized as a WW Health Coaching + Wellness program.

Weigh to Go! Health Coaching + Wellness Program Proposal

Statement of Purpose: The purpose is to promote an initiative for current WISEWOMAN participants to become an active member at the YMCA and participate in the Weigh to Go! weight loss program.

Benefits of Partnership: Weigh to Go! is a national program created by the YMCA that aims to help obese adults (BMI of 30 or higher required) to lose weight by adopting healthy lifestyle changes. This eight-week program connects WW clients to a certified wellness trainer that offers on-going nutritional and fitness support for practical behavior change. The program is delivered through one hour-long classes that occur once a week on weekday evenings. Success in the program is monitored through weekly weigh-ins, bi-weekly measurements, and results from two comprehensive fitness assessments delivered pre- and post-program. In addition to the Weigh to Go! program, WW will supplement a YMCA membership to provide clients with access to exercise equipment, thereby increasing adherence to the program regimen and promoting regular physical activity.

Proven Effectiveness: The YMCA of Indiana County, PA sponsors the Weigh to Go! program as a summer weight-loss initiative. Each year, the program has continued to prove itself effective as participants see substantial weight loss. For the previous season, the average weight loss for participants was 7 lbs. in 7 weeks.4

Goals of Weigh to Go! Program

  • Provide direct nutrition and fitness education through appointments with a certified wellness trainer.

  • Provide WW client with an on-going support network to encourage healthy lifestyle choices.

  • WW participants will develop an effective and sustainable exercise routine and adopt a healthier diet and see improvements in physical fitness and a reduction in BMI.

Goals of WISEWOMAN Supplements

  • Provide WW clients with a four-month membership to a local YMCA to encourage physical activity and healthier lifestyle choices.

  • Provide another community based health coaching option for WW clients to choose from.

  • Provide WW client with an on-going support network to encourage healthy lifestyle choices.

  • Establish proven effectiveness of WW partnership with YMCA Weigh to Go! program with the aim of implementing similar programs at additional YMCA locations.

  • Maintain WW clients’ active membership by providing them with connections to scholarship/reduced-fee membership rates upon conclusion of the Weigh to Go! program

Weigh to Go! HC+ Program Description

  • Clients must be eligible for Health Coaching (at least one abnormal value) and demonstrate to the Health Coach a willingness to participate in Weigh to Go! Program.

  • Health Coach will conduct monthly check-ins with the client to discuss weight loss, YMCA attendance, and barriers/success in health improvements.

  • YMCA certified wellness trainers will serve as the health coaches for the Jump Start program. Certified wellness trainers will offer the following services to WW clients

    • Weekly group workouts (1 or 2 per week)

    • Weekly weigh-ins and bi-weekly measurements

    • Nutrition education sessions

    • Pre and post program fitness assessments

    • Coaching and nutritionist support for developing practical behavior change strategies

    • Document WW client attendance.

  • Clients will be given tracker sheet to record each day they attend the YMCA for physical activity purposes and the type/rigor of exercise performed.

    • YMCA Staff member will review tracker sheet to ensure that what the client documents is accurate. This data will be essential to tracking the progress/effectiveness of the program.

  • YMCA will track WISEWOMAN clients’ attendance

Conclusion: While the Weigh to Go! program offered by the YMCA has led to health improvements for participants, this program framework must be supplemented in order to meet the required guidelines to be recognized as a WW Health Coaching + Wellness program. As described in this protocol, the Weigh To Go! program has previously been delivered in a 8-week framework. In order to meet the criteria of WISEWOMAN, a client must be provided with a 4-month membership to the YMCA at minimum. When this addition is added into the Weigh to Go! framework, then this program will be recognized as a WW Health Coaching + Wellness program.

Choose 2 Lose Health Coaching + Wellness Program Proposal

Statement of Purpose: The purpose is to promote an initiative for current WISEWOMAN participants to become an active member at the YMCA and participate in the Choose 2 Lose weight loss challenge.

Benefits of Partnership: Choose 2 Lose is a 16-week challenge created by the YMCA that motivates overweight adults to “lose weight, tone up, and make healthier choices”. Participants are organized into six trainer-led teams and receive a pre and post-fitness assessment, attend weekly trainer-led team meetings with health and fitness education classes, and receive on-going support and motivation throughout the program. Designed to help participants develop and commit to a new exercise routine, this program focuses on goal-setting to achieve long-term health success and provides incentives in the form of memberships and cash prizes for first, second, and third place (based on combined total % of body weight lost per team). In addition to the Choose to Lose challenge benefits, WW will supplement a YMCA membership to provide clients with access to exercise equipment, thereby increasing adherence to the program regimen and promoting regular physical activity.

Proven Effectiveness: The Choose 2 Lose challenge has been implemented at numerous YMCA facilities throughout the country with great success. Over the past 7 years, Choose 2 Lose challenges have helped communities lose over a total of ten tons.5 The Waynesboro Area YMCA in Pennsylvania sponsored this program back in 2013 experienced saw great success; the 50 participants lost over 500 lbs. combined and the winning team lost 12.7% of its total body weight.6 Detailed below are recommended guidelines for the Choose 2 Lose challenge tailored specifically for WW clients.

Goals of Choose 2 Lose Weight Loss Challenge

  • Provide direct nutrition and fitness education through weekly meetings led by a certified wellness trainer.

  • Provide WW client with an on-going support network to encourage healthy lifestyle choices.

Goals of WISEWOMAN Supplements

  • Provide WW clients with a four-month membership to a local YMCA to encourage physical activity and healthier lifestyle choices.

  • Provide another community based health coaching option for WW clients to choose from.

  • Provide WW client with an on-going support network to encourage healthy lifestyle choices.

  • Establish proven effectiveness of WW partnership with YMCA Choose 2 Lose challenge with the aim of implementing similar programs at additional YMCA locations.

  • Maintain WW clients’ active membership by providing them with connections to scholarship/reduced-fee membership rates upon conclusion of the Choose 2 Lose challenge

Choose 2 Lose HC+ Program Description

  • Clients must be eligible for Health Coaching (at least one abnormal value) and demonstrate to the Health Coach a willingness to participate in Choose 2 Lose challenge.

  • Health Coach will conduct monthly individualized check-ins with the client to discuss weight loss, YMCA attendance, and barriers/success in health improvements.

  • YMCA certified wellness trainers will serve as the health coaches for the Choose 2 Lose challenge. Certified wellness trainers will offer the following services to WW clients

    • Pre and post-fitness assessment

    • Weekly meetings, weigh-ins, and group workouts

    • My FitnessPal Orientation

    • Weekly goals and workout plan

    • Team challenges

    • Expert advice and ongoing-support for exercise, nutrition, and healthy living

    • Document WW client attendance in the Choose 2 Lose program

  • Clients will be given tracker sheet to record each day they attend the YMCA for physical activity purposes and the type/rigor of exercise performed.
