Jump Start Health Coaching + Wellness Program Proposal

Statement of Purpose: The purpose is to promote an initiative for current WISEWOMAN participants to become an active member at the YMCA and participate in the Jump Start wellness program.

Benefits of Partnership: Jump Start is an initiative created by the YMCA that combines the four core components to creating and maintaining an effective exercise regimen – personal fitness assessments, equipment training, nutrition education, and on-going support – into one wellness program. Through this program, YMCA certified Health and Wellness coaches will help participants achieve their personal fitness goals and develop permanent healthy lifestyle habits. In addition to the Jump Start program benefits, WW will supplement a four-month YMCA membership to provide clients with access to exercise equipment, thereby increasing adherence to the program regimen and promoting regular physical activity.

Proven Effectiveness: The Jump Start program is sponsored at numerous YMCA locations throughout the country. It offers a comprehensive introduction to healthier lifestyle change such as the adoption of a nutritious diet and commitment to a regular physical activity regimen. Research continuously proves that the most common barriers to health behavior change are an individual’s lack of education and motivation.7 The Jump Start program overcomes these challenges by connecting participants with certified personal trainers and Registered Dieticians who can provide both fitness and nutrition education while serving as an on-going support network assisting the participant as they embark on their quest for improved health.

Goals of Jump Start Program

  • Provide direct nutrition and fitness education through appointments with a Registered Dietician and a Personal Trainer.

  • Provide WW client with an on-going support network to encourage healthy lifestyle choices.

Goals of WW Supplementation

  • Provide WW clients with a membership to a local YMCA to encourage physical activity and healthier lifestyle choices.

  • Provide another community based health coaching option for WW clients to choose from.

  • Provide WW client with an on-going support network to encourage healthy lifestyle choices.

  • WW participants will develop an effective and sustainable exercise routine and adopt a healthier diet.

  • Establish proven effectiveness of WW partnership with YMCA Jump Start program with the aim of implementing similar programs at additional YMCA locations.

  • Maintain WW clients’ active membership by providing them with connections to scholarship/reduced-fee membership rates upon conclusion of Jump Start program

Jump Start HC+ Program Description

  • Clients must be eligible for Health Coaching (at least one abnormal value) and demonstrate to the Health Coach a willingness to participate in Jump Start Program.

  • Health Coach will conduct monthly check-ins with the client to discuss weight loss, YMCA attendance, and barriers/success in health improvements.

  • YMCA certified Health and Wellness Coaches will serve as the health coaches for the Jump Start program. Health and Wellness Coaches will offer the following services to WW clients

    • 2 Body Composition Analysis Tests

    • Exercise Equipment Orientation

    • Food Journal Review (from YMCA’s Registered Dietician)

    • Fitness Tracking Log

    • Personal Training

    • On-going support and counseling for maintaining regular exercise routine

    • Document WW client attendance

  • Clients will be given tracker sheet to record each day they attend the YMCA for physical activity purposes and the type/rigor of exercise performed.

    • YMCA Staff member will review tracker sheet to ensure that what the client documents is accurate. This data will be essential to tracking the progress/effectiveness of the program.

  • YMCA will track WISEWOMAN clients’ attendance.

    • When a client enters the facility and scans membership card date and time will be recorded.

    • Monthly YMCA attendance logs will be provided to WW program in order to have documentation of the days participants utilized membership.

  • Prior to completion of program, clients will be connected to resources of scholarship-based and reduced-fee memberships enabling them to continue membership after Jump Start program is completed.

Costs

  • The Jump Start program is offered free of charge for individuals with Adult YMCA membership. The Jump Start program includes 10% off personal training packages.

  • Joining Dues for Membership: Ask to be waived for WW client.

  • Monthly Dues for Membership: $30.00

  • Personal Training Fee: $30 per 30-45 minute session for 4 sessions = $120.00

Certified Health Coach (Personal Training + Nutrition Education) Health Coaching + Wellness Program Proposal

Statement of Purpose: The purpose is to promote an initiative for current WISEWOMAN participants to become an active member at the YMCA and participate in health coaching administered by YMCA certified health coaches, certified personal trainers, and/or certified nutritionists.

Benefits of Partnership: Not all YMCA locations have established wellness programs for WW clients to participate in, however, every YMCA franchise staffs employees who are certified to deliver health coaching. This proposal aims to connect WW clients with the on-staff certified health coaches, personal trainers, and/or nutritionists on a one-on-one basis so the client is able to receive the same wellness education that is offered by group programs. Individualized sessions with health coaches will focus on educating the client on how to increase physical activity and how to adopt a healthier diet as well as address any specific health needs of the client. WW will also supplement a minimum four-month YMCA membership to clients so they have access to exercise equipment. This framework is beneficial to the WW client because the sessions can be scheduled for a time that is convenient for her. Additionally, the personalized nature of a one-on-one setting will increase the client’s motivation and willingness to make healthy behavior changes.

Certified Health Coach HC+ Program Description

  • YMCA certified health coaches, personal trainers, and/or nutritionists will serve as the health coaches for the program. Health coaches will offer the following services to WW clients

    • Exercise Equipment Orientation

    • Nutrition Education

      • Nutrition education must focus on consumption of a whole foods diet consisting of fruits, vegetables, whole grains, lean proteins, and other heart healthy foods.

      • Nutrition education should NOT encourage clients to consume any supplements, pills, or powders for weight loss.

      • Health coach should develop meal plan for client that changes weekly to ensure the client has access to several healthy eating options suitable for her lifestyle.

    • Fitness Tracking Log

      • Health coach should develop an exercise regimen for the client to participate in throughout the duration of the program.

      • Exercise should include both cardio and strength training, as well as additional fitness classes the client wishes to participate in.

    • Personal Training

      • Health coach conducts personal training for up to 4, 30-45-minute sessions.

      • Personal trainer also provides an orientation to the gym to familiarize the client with exercise equipment.

    • On-going support and counseling for maintaining regular exercise routine and healthy diet.

    • Document WW client attendance

Pennsylvania WISEWOMAN Health Coaching + Fitness

Example of an Established Program

While it is ideal that the WW client participates in a Health Coaching + Wellness program to receive the most comprehensive health education, not all community establishments have the necessary infrastructure to provide the combination of physical activity and nutrition education. For this reason, this proposal includes the implementation of Health Coaching + Fitness programs at facilities which are only able to offer education on physical activity and access to exercise equipment.

The following is an example of a Health Coaching + Fitness program that WW clients would be approved to participate in. In the case that the selected Health Coaching + Fitness program does not meet all the requirements to be recognized by WW for approval, WW is able to fill in the gaps by supplementing additional health education resources paid for by the program’s funds.

Buhl Community Center Health Coaching + Fitness Program Proposal

Statement of Purpose: To provide WW clients residing near Buhl Community Center with an additional community resource which will promote their engagement in regular physical activity.

Benefits of Partnership: Conveniently located approximately 0.5 miles away from the Adagio Health Sharon site (87 Stambaugh Ave. Sharon, PA) where WISEWOMAN participants receive screening services, the Buhl Community Center is an ideal establishment for WW clients to gain membership to. The Buhl center has a full gym offering exercise equipment for both cardio and strength training. Group fitness classes such as indoor cycling, Zumba, Yoga, and others are offered on a weekly basis free to members. The Buhl Community Center also has a pool which members can take advantage of for free swimming or for any of the group aquatic classes offered. Personal Training is available as a one-on-one basis as well. The Buhl Community Center offers sliding fee rates for low-income individuals; this is imperative for our clients who wish to continue membership after the program is complete.

Goals of Buhl Community Center Health Coaching + Fitness Program

  • Provide WW clients with an additional community resource that promotes physical activity

Buhl Community Center HC+ Description

  • Clients must be eligible for Health Coaching (at least one abnormal value) and demonstrate to the Health Coach a willingness to participate in the Health Coaching + program at Buhl Community Center.

  • WW will pay for a minimum four-month membership to Buhl Community Center for the WW client to provide her with access to exercise equipment, thereby promoting physical activity.

  • WW will pay for up to 4, 30-45 minute personal training sessions.

    • Personal trainer will conduct pre- and post-fitness assessments measuring body weight, body composition (BMI), cardio fitness, strength fitness, blood pressure, waist to hip circumference

    • Personal trainer will provide gym orientation so WW client is familiar with all exercise equipment and informed about all the classes available to her through her membership.

    • Personal trainer will work with WW client to develop a daily exercise routine.

    • Personal trainer will review activity tracker log to ensure that client is reporting accurate exercise. This is imperative to determining the success of the program.

  • Buhl Community Center is responsible for tracking attendance of WW client in the manner they deem most fit. Monthly attendance logs are to be provided to WW program.

  • WW program health coaches will provide follow-up calls with participants at the beginning and end of the program as well as once a month throughout to gather feedback on client satisfaction and participation. These phone calls will also address any questions/concerns the client may have in an effort to sustain her engagement in the program.

Cost

  • Enrollment Fee: Ask to be waived for WW client

  • Monthly membership: $37 per month for minimum four-months = $148

  • Personal Training Sessions: $35 per session for four sessions = $140

  • Health Coaching Calls : $25 per call for up to four calls = $100
