Targeted Case Manager (TCM) received a phone call from Marisol where she referred that she needs to obtain low cost Internet because her income is very low to pay for the regular price of this service and she is interested in this option in order to reduce her monthly expenses in financial obligations. Marisol is unable to apply for this service by herself due to her mental condition with symptoms such as lack of energy, poor concentration, and nervousness, for that reason, TCM explained Marisol that Xfinity is a program from government that help low income people to obtain a credit for internet services and she would receive assistance to select a plan according to her need and preference. Marisol asked TCM how to apply for such credit and asked TCM assistance for it, TCM agreed and replied that she would be assisted to complete the required documentation and its requirements and discuss with her at library about it the same day. Marisol thanked TCM.
2. Targeted Case Manager (TCM) met Marisol at Library to assist her during the whole application process for an Affordable Connectivity Program (ACP) credit from Xfinity Internet Service. Marisol requires guidance and assistance to deal with this kind of tasks because her mental symptoms had affected her level of functioning. After greetings they entered the center and
TCM explained that the first step must be to verify Marisol’s ACP eligibility. TCM opened her laptop and entered the website https://www.usac.org-lifeine-nationalverifier. TCM the indicated screen instructions. TCM could access a form where they had to provide Marisol’s full name, DOB, address, SSN, ID number, and others. It was also necessary to mark the type of assistance that Marisol was currently receiving from federal programs. Once the document was completed Marisol and TCM made sure that all information was correct and submitted it. Immediately the website sent a notification of Marisol’s approval for the ACP credit, as well as a Xfinity ID code required to reflect in the first enrollment form. Then, TCM referred that they would complete that form, and accessed the website https://wwwfcc.gov. TCM linked in apply now option and was able to locate the enrollment form. First of all, it was requested to write the Xfinity ID code received from the national verifier website. TCM advocated for Marisol writing down such code, and continued providing name, address, DOB, etc. They made sure that Marisol’s personal data matched with the submitted one in the National Verifier form. When this form was filled out, TCM and Marisol completed also an inscription form with demographic and contact information from Marisol SSN, her household incomes, etc. They also selected the internet company which Marisol was linked with
(Xfinity/Comcast). Marisol had to select one of the three Xfinitty essential credit plans, the wired $30.00 credit plan in this case. When they finished TCM and Marisol reviewed the document and submitted it. Immediately the website sent a phone number from Xfinity/Comcast representative to attend a phone interview as last part of the process.
TCM and Marisol arranged to perform a three-way call to contact with the Xfinity/Comcast representative next day given the time expended during the completion of the forms.
TCM assisted Marisol to verify her ACP eligibility and to complete the enrollment and inscription forms for ACP credit from Xfinity Internet Service.
TCM and Marisol will perform Threeway
Xfinity/Comcast request credit.
