EVALUATION CRITERIA:

  1. General considerations:



  1. The Department of Veterans Affairs will implement the Veterans First Contracting Program P.L. 109-461 and the VA Rule of Two – means the process in 38 U.S.C. 8127 (d) whereby a contracting officer of the Department “shall award contracts on the basis of competition restricted to small business concerns owned and controlled by veterans, if the contracting officer has a reasonable expectation that two or more small business concerns owned and controlled by veterans will submit offers, and that the award can be made at a fair and reasonable price that offers the best value to the United States.” Preference will be given to Veterans-Owned Small Businesses in accordance with above mentioned implementation. Offerors shall be registered in VetBiz at the time of award. In addition, any large business submitting an offer on this solicitation is highly encouraged to hire or subcontract veterans in the performing of this contract.



  1. The Government will determine acceptability by assessing the offeror’s compliance with the terms of the Request for Quotations to include the technical information required by the Performance Work Statement (PWS) on Attachment A.



  1. The Government will evaluate the quotations received as per FAR 13.106-2.

    1. Quotations will be evaluated in an impartial manner.

    2. Contracting will conduct a comparative evaluation of offers.

    3. All quotations will be considered; however those offers having the highest past performance rating will represent the best value, when compared to any lower priced quotation/offer.

    4. Quotations/Offers in response to this solicitation shall be provided via postal mail or in person to:

    1. Please DO NOT submits offers electronically via email because of limitations on transmitting data. However, if you determine to submit it electronically, the Government will not be responsible to any non-transmitted data or documents. Make sure that all submitted information is received in our office by the closing date and time.

    2. A comprehensive paragraph addressing ALL the elements in each of the evaluation factors (Four as below mentioned) will be appropriate; except when required to list or mention specifics; such as the three (3) references that shall be completely listed with all information; as requested. The company capabilities statement or brochures can be submitted, but will not be considered as a response to the evaluation factors.

    3. Submit with your Offer or Quotation any plan to sub-contracting; especially when hiring veterans or small businesses.

    4. Offeror shall submit the Price or Cost in a separate folder or package to the Evaluation Factors or Technical Submittal.

    5. Offers/Quotations shall be submitted by or before Wednesday, May 10, 2017 and not later than 2:00 PM Atlantic Standard Time.

    6. Questions on this procurement are due on Monday May 1, 2017 at 2:00 PM Atlantic Standard Time- Puerto Rico local time.



  1. At minimum, the Government will determine contractor’s responsibility by analyzing whether the apparent successful offeror complies with the requirements of FAR 9.1 including, but not limited to:



  1. Adequate financial resources or the ability to obtain them;



  1. Ability to comply with the required performance period, taking into consideration all existing commercial and governmental business commitments;



  1. Satisfactory record of integrity and business ethics;



  1. Necessary organization, experience, and skills or the ability to obtain them;



  1. Necessary equipment and facilities or the ability to obtain them; and



  1. Be otherwise qualified and eligible to receive an award under applicable laws and regulations.

EVALUATION FACTORS:


  1. Key Personnel Qualifications



    1. Service personnel must be Biomedical Engineering Technicians, Biomedical Engineers or an Electronics Technician with experience in medical equipment and knowledge in human anatomy and human physiology. Evidence of compliance is required.

    2. Service personnel must have experience in medical equipment of the same manufacturer and model as the ones included in this service contract. Evidence of compliance is required.

    3. Offeror must possess all necessary licenses and access for support from equipment manufacturer to service and maintain the equipment included in this contract. Evidence of compliance is required.

    4. Service personnel must have been trained and certified for service and maintenance of the equipment models included in this service contract.



  1. Technical Approach/Technical Capabilities



    1. Proposed Methodology

      1. Offeror must provide the list of employees responsible to complete the repair and maintenance services requested in this contract.

      2. Offeror must provide a schedule detailing the dates and starting time and estimated ending time at which the requested preventive maintenance services will be provided and the service personnel responsible for each service.

      3. Offeror must own all necessary diagnostic tools for all equipment models included in this service contract, e.g. Somatom Definition Flash, Somatom Definition AS20, Ysio, Biograph TruePoint 64.

    2. Management Plan

      1. Offeror shall provide action plan addressing compliance with Government requirements.  Plan shall demonstrate how the Contractor will ensure services will be provided in accordance with Government requirements, quality standards and established business practices.

      2. Offeror must have experience in managing a similar contract at a medical facility institution accredited by The Joint Commission.

    3. Knowledge and Task Understanding

      1. Offeror must provide the checklist for the performance verification for each model of medical equipment units included in this service contract

      2. Offeror must provide the preventive maintenance procedure to be completed for each model of the medical equipment included in this service contract.

      3. Offeror must possess a complete service manual and parts list for each model of medical equipment included in this service contract.

      4. Service personnel must have the capacity to install all updates, field corrective actions/options required by original equipment manufacturer or agencies such as FDA.



  1. Past Performance



    1. Past Performance must be relevant and recent. In order to be considered relevant and recent, references shall be similar in nature to the Government requirement, and services performed within the two years preceding the submission of this offer.

    2. Offerors shall provide three (3) references of past or current contracts. References may include federal, state, local government and/or private sector. References shall include company name, point of contact, telephone number, contract amount and performance period and equipment description (manufacturer, model)

    3. Past Performance information to be evaluated includes:

      1. Experience in medical equipment of the same type, manufacturer and model as the units included in this service contract.

      2. Quality of Service (contractor performance in making sure the service conforms to contractual specifications).

      3. Timeliness (offeror adherence to contract schedules).

      4. Describe any difficulties and obstacles encountered during performance of the contract and the steps taken to resolve these issues.

      5. Identify key personnel and their duties and responsibilities.

      6. Describe any unforeseen conditions and how were they resolved.



  1. Price/Cost



      1. Fair and reasonable price shall be submitted to the RFQ response. The price submitted on the quotation shall be the optimal and final one for these services. Offerors shall quote each item listed on the Schedule of Prices.

      2. If partnering, joint venture, or any other type of subcontracting method, a percentage of work to be performed in cost by each of the parties shall be submitted with the offer/quotation. If SDVOSB or VOSB: For service contracts, the VOSB or SDVOSB generally has requirements to incur at least 50 percent of the cost of the contract using its own employees. The name of the subcontractor shall be provided to ensure compliance with SAM registration. In addition, the agreement between both parties shall be submitted with the quotation to be evaluated by the Government with the offer; prior to award.
