Maintenance Supply Contracts only
Hot mix asphalt and cold plant mix asphalt furnished for maintenance supply contracts may be accepted based on supplier certification, providing that the supplier agrees to the following:
Perform all quality control testing according to the current edition of the Department’s Standard Specifications for HIGHWAY CONSTRUCTION;
Perform sampling and testing at a minimum frequency of one test for each 750 metric tons (750 tons) produced by the plant for AHTD maintenance or construction projects during the period of the maintenance supply contract;
Test hot mix and cold mix for conformance with the current edition of the Department’s Standard Specifications for HIGHWAY CONSTRUCTION;, applicable Supplemental Specifications and Special Provisions;
Maintain plot of all test results by mix type for all mixes produced for AHTD construction projects and the maintenance supply contract during the period of the Supply Contract;
Maintain and make available for review by the District Materials Supervisor one copy of all quality control test results for the duration of the Supply Contract;
Immediately notify the Department’s District Materials Supervisor of non-complying material produced for either AHTD construction projects or the maintenance supply contract, identify extent of the problem, and initiate corrective action to bring the mix into compliance;

Submit with each invoice a listing of the tonnage produced from each accepted mix design and include the following statement signed by an authorized representative of the company:
“ This is to certify that the Hot Mix Asphalt Cold Mix listed herein produced by (Company Name) at (Plant Location) was produced from a mix design that was accepted by AHTD; that Quality Control Testing has been performed at the frequency stated in the certification agreement, and that the results of the tests conform to the requirements of the specifications.”
The Department will visit the producer’s facility as necessary to review the quality control program and/or sample mix for verification testing.
Acceptance by this method is subject to price adjustments as per Subsection 410.09 of the current edition of the Department’s Standard Specifications for HIGHWAY CONSTRUCTION. This Certification Agreement may be withdrawn if material fails to comply with requirements and samples continue to indicate that sufficient adjustments are not made to correct the deficiency. If certification acceptance is withdrawn, it will be necessary that material be sampled, tested, and the results reported according to the procedures in Section 410, Section 411 or other applicable specifications of the current edition of the Department’s Standard Specifications for HIGHWAY CONSTRUCTION.
