This application is a permanent record. All information must be typewritten and printed on single-sided pages. Applications that are handwritten, incomplete, or printed on double-sided pages will not be accepted.

APPLICANT INFORMATION

The information you provide in this personal history statement will be used in the investigation to assist in determining your suitability for a position of trust within the Sacramento County Sheriff's Department. Please fill out the questionnaire completely and accurately. Keep in mind that:


  1. All statements are subject to verification;


  2. Deliberate inaccuracies or incomplete statements may bar or remove you from employment;


  3. All time periods in your personal history must be accounted for.

It is to your advantage to respond openly. Any negative factor in your background will be evaluated in terms of the circumstances and facts surrounding its occurrence, and its degree of relevance to the job. For example, being fired from a job or having an arrest record is not in itself grounds for disqualification. During the investigation, the investigator will inquire into the facts surrounding such an occurrence. An evaluation will then be made of the relevance of these facts to the requirements of the job.


The Personal History Statement and Personal Data form must be typewritten. In your Personal History Statement, the only acceptable handwritten entries are your initials in the space provided at the bottom of each page, and your signature and date on the last page. All other attached forms, e.g., waivers, releases, advisements, etc., may be handwritten.
If a question does not apply to you, write N/A (not applicable) in the space provided for your answer. If you cannot obtain or remember certain information, indicate so in your response. If you need more space to respond to a question, use the last page of this form (page 26) and identify the additional information by question number. If you use page 26 or additional pages to respond to a question, it is important that you include all information as required by the specific question. For example, for additional employers, the required information is employer name, address including zip code, job title and duties, dates of employment, first and second level supervisors, the names of three co-workers, reason for leaving, etc.
Disqualification:
There are very few automatic bases for rejection. Even issues of prior misconduct, such as prior illegal drug use, driving under the influence, theft or even arrest or conviction are usually not, in and of themselves, automatically disqualifying. However, deliberate misstatements or omissions can and often will result in your application being rejected, regardless of the nature or reason for the misstatements/omissions. In fact, the number one reason individuals “fail” background investigations is because they deliberately withhold or misrepresent job-relevant information from their prospective employer.

BOTTOM LINE: Be as complete, honest and specific as possible in your responses.
