Parents should notify the office of changes in home address or phone numbers. It is particularly important thatwe have current phone numbers listed for student emergencies.

Chaperones volunteering to attend off-campus activities must have a completed criminal history background check report (Auxiliary Volunteer form) on file at the District Office prior to performing auxiliary volunteer activities. Forms are available from the teacher or the school office. There is a processing
fee of $22.50 and completion needs to be done only once during your child’s K-12 career.
GUIDELINES
Set a good example for the students in your group.
Insist upon good behavior from your students.
Be in control of your students and be prepared to handle discipline type situations.
Supervise your students at all times and make sure students treat the field trip destination with respect.
Use of profanity by chaperones and students is prohibited.
The Columbus Character Campaign is a school-wide effort to build students with good character. We have identified eight traits for emphasis—responsibility, kindness, perseverance, respect, self-control, citizenship, tolerance and honesty. Each school year we promote four traits and then rotate to the other four traits the following school year. Banners of these eight traits hang in our media center.
