CHANGE OF ADDRESS OR PHONE NUMBERParents should notify the office of changes in home address or phone numbers. It is particularly important that we have current phone numbers listed for student emergencies.CHAPERONE POLICY – see District policy #438Chaperones volunteering to attend off-campus activities must have a completed criminal history background check report (Auxiliary Volunteer form) on file at the District Office prior to performing auxiliary volunteer activities. Forms are available from the teacher or the school office. There is a processing fee of $22.50 and completion needs to be done only once during your child’s K-12 career. GUIDELINESSet a good example for the students in your group. Insist upon good behavior from your students. Be in control of your students and be prepared to handle discipline type situations. Supervise your students at all times and make sure students treat the field trip destination with respect. Use of profanity by chaperones and students is prohibited. No smoking. Use cell phones only in case of an emergency. Individual grade levels will determine the number of chaperones. Model positive language and dress style. No preschool siblings. Please ride the bus(es) provided. Adult chaperone must be a member of the immediate family. CHAPERONE DISCIPLINE GUIDELINESNo physical or verbal abuse will be TOLERATED. Maintain a respectful dialogue with the student(s) and reiterate the rules of acceptable behavior. If a problem persists, refer student(s) to the teacher. CHARACTER CAMPAIGN
