portland cement is tested, certified, and accepted in accordance with certified supplier procedures. All portland cement manufacturing plants and their location/s should be identified on the certified shipping notice.
METHOD OF DOCUMENTATION OF USE:

The inspector shall document on the REPORT OF TESTS OF CONCRETE CYLINDERS the name of the producer/supplier, source location, and the type cement.


PROCEDURE AGREEMENT: The listed manufacturers are approved to ship cement in accord with the attached agreement.
Destination Sampling and Testing:

Materials Division will request random samples through the Resident Engineer. These random samples shall be taken from the transport trailer/railroad car and be identified by:

A. Name of producer/supplier/terminal/plant

B. Cement Type

C. Bill of Lading Number

D. Producer/Supplier Bin (silo) Number

E. Mill Test Number

AGREEMENT FOR THE ACCEPTANCE

OF CEMENT BASED ON CERTIFIED MILL TESTS

Portland cement will be accepted by the Arkansas State Highway and Transportation Department based on the producer's certification that the material meets all requirements of applicable Department specifications (AASHTO M 85)under the following conditions:

1. The producers shall maintain a file of their test results and methods of testing. The producer's laboratory shall be inspected periodically by the Concrete and Cement Reference Laboratory of the National Institute of Standards and Technology, or other acceptable testing laboratory, and upon request, make reports of inspections available to the Department.

2. Mill analyses covering the cement to be certified and shipped to Department projects shall be furnished to the Department's Materials Division. Mill analysis must reference plant of origin.

3. Shipments to Department projects shall be made only from bins (silos) that have been tested and found to meet Department specifications. No cement shall be shipped prior to satisfactory completion of three day compressive strength tests. Cement from different manufacturing plants will not be intermixed at terminals.

4. The producer shall furnish with each shipment a certification containing the following:

a. consignee

b. date and time of shipment

c. truck or railroad car number

d. quantity of cement shipped

e. type of cement

f. bin (silo) number from which shipped

g. a statement that "This is to certify that the type cement in

this shipment is from pretested bin (silo) number___________ and complies with the AASHTO specification for the type shipped."

h. name of cement plant of origin, terminal (if applicable) and the

signature of a responsible company official

One copy of the shipment certification form for the Resident Engineer shall accompany the shipment.

5. A monthly composite sample will be sent to the Materials Division. This

monthly composite sample will be composed of approximately equally sized cement samples obtained from pre­tested bins. A composite sample will be sent for each type of cement supplied to Department projects. Composite samples will be supplied from the manufacturing plant, not from terminals.

6. Destination samples will also be taken as deemed necessary to assure compliance with specifications. In the event samples taken at the job fail to meet specifications, the Department will reject the cement. No further shipments will be accepted under this agreement until tests by the Department determine that the producer's product is in compliance with applicable specifications and requirements.

Acceptance of cement by this method is subject to withdrawal for failure to comply with the above requirements.

Please acknowledge agreement to the above requirements by signing and returning a copy of this letter for our producer files.

REQUIREMENTS FOR CONCRETE AIR-ENTRAINING AGENTS
The following procedure is to be used in acquiring approval of air-entraining agents:
1. The manufacturer/supplier shall provide the Materials Division with a test report from a recognized laboratory indicating compliance with AASHTO M 154, Standard Specification for Air-Entraining Admixtures for Concrete. A recognized laboratory is any State Highway Agency, Federal Highway Administration or independent laboratory regularly inspected by the National Institute of Standards and Technology. All test reports shall include the manufacturer’s name, brand name of the material, and date of testing.
2. In addition to the requirements of the AASHTO Specification, the manufacturer/supplier shall include:
a. The constituents of the admixture. If the admixture is manufactured by neutralizing Vinsol resin with caustic soda (sodium hydroxide), the ratio of sodium hydroxide to Vinsol resin shall be given.

b. The chloride content of the admixture and whether or not chloride was added during its manufacture.

c. Uniformity tests according to AASHTO M 154 to establish compositional or chemical equivalence. Where the nature of the admixture or the Department’s testing capabilities may make some or all of the procedures unsuitable, other requirements may be established by agreement between the Department and the manufacturer.

d. Recommended dosage rate.


3. A sample of the air-entraining agent will be supplied to the Materials Division at the following address. Product information and Material Safety Data Sheets must be included.

4. If the product meets material requirements, the product will be approved for inclusion after receipt of a signed QPL certification.


5. Destination samples will be taken as deemed necessary to assure compliance with specifications. Material failures either in the laboratory or in field applications may be considered sufficient cause to reject the material. The Materials Engineer will determine if the failures warrant discontinuing acceptance and removal from this QPL.
