The process for State Agencies to purchase cellular service, telephones or accessories from Alltel requires the following:
1. A purchase order and Cellular Authorization Form that includes

Bill to address.

Ship to address and contact person.

Existing account number or mobile number if adding new lines.
Or,
A Cellular Authorization Form signed by a purchasing agent, telecommunications manager or other authorized signer.

Agencies may use the bill to airtime account procedure (when purchasing equipment or accessories), if the Cellular Authorization Form states that is the preference. Please include the cellular airtime account number and mobile number that items are to be billed to.

State Purchasing Cards are accepted with the Cellular Authorization Form.

AMENDMENT #1
TO CONTRACT 82060
BETWEEN
THE STATE OF WISCONSIN
DEPARTMENT OF ADMINISTRATION
AND
ALLTEL COMMUNICATIONS
This amendment is for the purpose of adjusting the State’s cellular rate plan as proposed in the following documentation. The new State cellular rate plan, including terms and conditions of the existing contract, shall be available to “Authorized Users” which are “municipalities” as defined in Wisconsin State Statute 16.70(8), including counties, cities, villages, towns, school districts, board of school directors, sewer districts, drainage districts, technical college districts or any other public or quasi-public corporation, officer, board or other body having the authority to award public contracts.
Secondly, the State hereby exercises its right (per contract section 1.2.3.6) to invoke the available optional two-year contract extension and extend the contract term to April 30, 2007.
This Amendment to the Contract is complete and valid as of the date signed by the Secretary of the Department of Administration.

Proposal For State Rate Plan
This proposal, offered by Alltel Communications to the State of Wisconsin, contains details of amendments to the cellular service rates previously offered by Alltel Communications. This rate is only available to State of Wisconsin Agencies and Authorized Users within Alltel Communications operated territories in Wisconsin.
This proposal outlines amendments to the current State of Wisconsin rate plans as originally defined in section 6.0 of the current contract for cellular services between Alltel Communications and the State of Wisconsin. It includes an adjusted State Rate Plan for monthly service with additional detailed information about vendor provided basic equipment package, provisions for RSA 3, border tower billing and alternate rate plans.
