All accounts using current State of Wisconsin Rate Plans ($9.99/month + 30 minutes, $21.99/month + 180 minutes, $27.99/month + 300 minutes) shall be converted to the Amended State of Wisconsin Rate Plan. Current State of Wisconsin Rate Plans shall be eliminated following completion of the conversion. The Alltel provided State of Wisconsin web site shall be updated to reflect the rate plan changes.

Local Alltel Business Solutions Representatives shall be educated on the details of the State of Wisconsin’s contract with Alltel Communications so assistance to state agencies and authorized users may be provided.
BASIC EQUIPMENT PACKAGE OFFER

Replace current Nokia 5165 TDMA equipment with Nokia 3587i CDMA equipment.

Immediately begin Year 4 Equipment Replacement as defined in Section 5.5.2, scheduling one agency at a time with goal to complete CDMA conversion by May 1, 2005.
The Alltel provided State of Wisconsin web site shall be updated to reflect basic equipment package changes.
RURAL SERVICE AREA (RSA) 3 – Formerly CellCom
Vilas, Oneida, Florence, Lincoln, Langlade, Forest and Taylor counties in Northeast Region

Alltel Communications shall:

Build the State and alternate rate plan structures in the RSA3 billing system.
-
Identify existing State and Authorized Lines utilizing Alltel service.
-
Convert lines to State or alternate rate plan.
-
Implement and honor all terms of the existing and amended contract.
AUTHORIZED USERS
Wisconsin State Statute 16.70(8) defines authorized users a “municipality” meaning a county, city, village, town, school district, board of school directors, sewer district, drainage district, technical college district or any other public or quasi-public corporation, officer, board or other body having the authority to award public contracts. Authorized users currently subscribing to Alltel wireless service, not purchasing from the State’s contract, may opt to convert to this proposed State Rate Plan that includes all terms of the State’s contract with Alltel Communications. Applicable break contract fees shall be waived.
Authorized users currently purchasing wireless service from the State’s contract shall be required to convert to the proposed State Rate Plan options as described in this document, Amendment 1.

Wireless service purchased from the State of Wisconsin’s contract with Alltel Communications is intended for official business use only. Employees, their families or any other business a user may be associated with may not be extended this plan for personal use.
“BLEED OVER” BORDER TOWERS
Alltel Communications shall add the following border towers into billing systems to automatically void interstate roaming charges incurred when travelling inside the borders of Wisconsin but receiving radio frequency signal from outside the State.

Alltel Communications – Ironwood, Michigan
Alltel Communications shall work with agencies and authorized users to identify new “bleed over” roaming areas. Alltel shall proactively work with their engineering department to identify these areas as well. Should “bleed over” tower roaming charges appear Alltel shall escalate addition of the towers to their billing systems as well as provide appropriate credits.
ALTERNATE RATE PLANS
A 15% discount shall be applied to alternate rate plans that may more economically satisfy user requirements. The discount will be applied to the primary line only when sharing the minutes on an alternate rate plan. All other terms of the existing and amended contracts shall apply. Current State of Wisconsin Alternate Rate Plan users shall receive the 15% discount. The Alltel provided State of Wisconsin web site shall be updated to reflect the alternate rate plan changes.
IMPLEMENTATION
Alltel Communications shall develop and present the State an implementation proposal. Official implementation of the agreement resulting from this Amendment shall begin October 1, 2004.
THE STATE OF WISCONSIN AGREES TO

Invoke the optional 2-year contract extension, included in the current contract, to April 30, 2007.

Encourage State Agencies and Local Governments (authorized users) to consolidate bills into a minimal number of accounts.

Grant Alltel Communications secondary provider status in the Northeast Region for RSA3 (Vilas, Oneida, Florence, Lincoln, Langlade, Forest and Taylor Counties).

Communicate contract Amendment 1 details to State Agencies and Local Governments (authorized users).

Communicate that wireless service purchased from the State of Wisconsin’s contract with Alltel Communications is intended for official business use only. Employees, their families or any other business a user may be associated with may not be extended this plan for personal use.

Procurement Process
The process for State Agencies to purchase cellular service, telephones or accessories from CellularOne requires one of the following:
1. A purchase order, with the following information:

Existing account number or mobile number if adding new lines.
Or,

A Cellular Authorization Form signed by a purchasing agent, telecommunications manager or other authorized signer.

Agencies may use the bill to airtime account procedure (when purchasing equipment or accessories), if the Cellular Authorization Form states that is the preference. Please include the cellular account number or mobile number that items are to be billed to.
