Billing AND PAYMENT Parameters

U.S. Cellular will invoice each Authorized User entity with a single bill mailed to a single business address. Each Authorized User entity will issue a single check for payment of the invoice.
THE STATE OF WISCONSIN AGREES TO

  Invoke the optional 2-year contract extension, included in the current contract.

  Set the expectation that ALL Authorized Users within U. S. Cellular territories use this program.
  
• 
  Agree the Authorized User program be available for official business use only. Employees, their families or any other business they may be associated with may not be extended this plan for personal use.
