AUTHORIZED USER AMENDED RATE PLAN


  • Monthly Access Charge: $4.75 PER MONTH PER LINE

  • Rate per Minute: $0.09 per minute

  • Home Footprint: State or Wisconsin (excluding St. Croix County)

  • Roaming Charge: $0.75 / Minute in U.S.

  • Toll/Long Distance: Included

  • Activation Fee: Waived

  • USF: Standard rate charge subject to change during the life of the agreement.

  • Term of Agreement: Co-Terminus with State of Wisconsin contract

  • Contract Break Fee: $150.00 (pro-rated over time of contract). The break fee will be divided

by the time left on the contract from activation date. The remaining months will be multiplied by the monthly break fee cost to reach the actual break fee. An example to this would be: Authorized user activates service in January 2005. Contract end date is April 30, 2007. This leaves 27 months of time on the contract. $150.00 / 27 months = $5.55 per month. If customer cancels service in November 2006 they will pay $5.55 per month for the remaining 5 months.

  • Liquidated Damages: Authorized Users who activate service during the final 12 months of

the State’s contract and then choose to leave U.S. Cellular at the end of the contract period will pay liquidated damages for Equipment. Liquidated Damages will be assessed based on calculating the difference between U.S. Cellular list price for the equipment provided and what the Authorized User actually paid for the equipment.

EQUIPMENT OFFER


  • Authorized user will own the phone and accessories.

  • Each new phone has a 12-month manufacturers warranty.

  • Equipment repairs and replacement will be at Authorized Users expense.

  • Phone package includes: Phone, Standard Battery and A/C Charger.

Cost: $19.95 for basic phone at activation.

For Higher end phones- a $110.00 credit off of suggested Retail price list at activation.

Optional accessories will be available at standard accessory pricing.

PHONE UPGRADES


  • Phones are eligible for upgrade at the end of the initial 24 months of service.

  • Cost for upgrades are as follows:

$19.95 for basic phone available at time of upgrade.

For Higher end phones- a $110.00 credit off of suggested Retail price list at time of upgrade.

Optional accessories will be available at standard accessory pricing.


  • Upgrade requests will be facilitated by the local field support representative.


IMPLEMENTATION
Official implementation of the Authorized User agreement resulting from this Amendment will begin February 15, 2004.

Billing AND PAYMENT Parameters


U.S. Cellular will invoice each Authorized User entity with a single bill mailed to a single business address. Each Authorized User entity will issue a single check for payment of the invoice.
THE STATE OF WISCONSIN AGREES TO


  • Invoke the optional 2-year contract extension, included in the current contract.

  • Set the expectation that ALL Authorized Users within U. S. Cellular territories use this program.

  • Agree the Authorized User program be available for official business use only. Employees, their families or any other business they may be associated with may not be extended this plan for personal use.
