Major Account status requires that arrangements for repair of phones and accessories be made directly with a U.S. Cellular Inventory Specialist.

This is the Authorized Process for repair / replacement of cellular phones and accessories:

  USCC Owned: Nokia 5165 or 3586, battery, ear bud headset, AC Wall Charger (originally packaged in the Nokia phone box), cigarette adapter, and leather case provided at contract implementation (5/1/2002 or after)
  
• 
  State of WI Owned: All other cellular phones and accessories
  

• 
  If necessary, call the U.S. Cellular Inventory Specialist for phone type and ESN location; and for State Owned phones, the warranty status. 608- 441- 4525

This amendment is for the purpose of clarification and correction of two items.

1. 
   This item clarifies the final agreement between the parties for the understanding of the intent of Section 5.9.1, Maintenance, of the originalRFP-3066. Section 5.9.1 is amended to read:
   

Contractor shall make any necessary repairs or replacement of vendor owned equipment, regardless of cause, at no additional cost to the State. The State shall pay all reasonable costs in the case of recurring neglect or abuse of equipment that is mutually agreed upon by both parties. Contractor shall provide any necessary repairs or replacement of State owned equipment no longer under manufacturer warranty for a charge not to exceed current consumer maintenance rates. Contractor shall be responsible for shipping and delivery charges regardless of ownership.

2. 
   This item corrects an error in Appendix A, Rate Plan of the Contract. The meeting of the minds on InterState Long Distance was no charge for that feature. Replace the original copy of the rate plan with the revised copy attached herein.
   

This Amendment to the Contract is complete and valid as of the date signed by the Secretary of the Department of Electronic Government.

This amendment is for the purpose of extending the State’s cellular rate plan of $4.75 per month access, $.09 per minute usage, $0.00 per minute long distance, $0.75 per minute interstate roaming and $0.00 per minute intrastate roaming (excluding St. Croix County) to Authorized Users as defined in Wisconsin State Statute 16.70(8). Municipalities included are counties, cities, villages, towns, school districts, board of school directors, sewer districts, drainage districts, technical college districts or any other public or quasi-public corporation, officer, board or other body having the authority to award public contracts.
Secondly, the State is exercising its right (per section III C) to invoke the available optional two-year contract extension and extend the contract term to April 30, 2007.
This Amendment to the Contract is complete and valid as of the date signed by the Secretary of the Department of Administration.

Proposal For Authorized User Rate Plan

This proposal, offered by U.S. Cellular to the State of Wisconsin, contains details of amendments to the cellular service rate previously offered by U.S. Cellular to State of Wisconsin Authorized Users within the State. This rate is only available to Authorized Users within U.S. Cellular operated territories in Wisconsin.

Authorized Users are identified per state statute as the following: (8) "Municipality" means a county, city, village, town, school district, board of school directors, sewer district, drainage district, technical college district or any other public or quasi-public corporation, officer, board or other body having the authority to award public contracts.
This proposal outlines amendments to the current State of Wisconsin Authorized User program as defined under section 6.4 of the current contract for cellular services between U.S. Cellular and the State of Wisconsin. It includes an adjusted Authorized User rate for monthly service with details on the included features as identified under section 5.11. This document also describes equipment options for Authorized Users, accessory purchases, ownership, warranty, repair/replacement and upgrade options, as these pertain to section 5.5 of the State contract.
