This template constitutes the Stormwater Management Plan (SWMP) for the facility, when completed and signed by the owner or operator of a facility that meets the applicability criteria in the Colorado Department of Public Health and Environment, Water Quality Control Division (WQCD) CDPHE General Permit COR900000. This template addresses the requirements of the General Permit and includes information specific to this facility. A complete copy will be maintained at the facility and not necessary to file a copy to COPHE. Facility Information:
Self-Certification Statement: The owner or operator of a facility certifies that each of the following is true in order to utilize this template to comply with the SPCC requirements:
I am familiar with the applicable requirements of the CDPS General Permit COR900000. I have visited and examined the facility; This plan was prepared in accordance with accepted and sound industry practices and standards; Procedures for required inspections and testing have been established in accordance with the requirements outlined in the General Permit; This Plan and individual(s) responsible for implementing this Plan have the full approval of management and I have committed the necessary resources to fully implement this Plan. Amendments to the plan will be documented as outlined in this SWMP.
