  1. Meeting called to order at 9:03 am by committee. The meeting was held at F.A.C.E.S. Community Services at 3500 Tejo Lane, Suite 105, Chesapeake, VA 23321.




  1. Adoption of Agenda: The LHRC voted to adopt the meeting agenda.



  1. Approval of Minutes:

The LHRC motioned, seconded and voted to approved the minutes as submitted. from the meeting on 10/27/2014 after making the following modifications:

    1. Genuine Heats Community Care (residential Group home)

The LHRC motioned, voted and approve the program’s affiliation with the LHRC.



  1. Old Business:

  1. Superior Community Services, LLC closing 4 Castle Haven Road Hampton, Va. 23666

  2. Harmony House, Inc. closing two locations 119 Patrician Drive Hampton, Va. and 2 Battle Road Hampton, Va. Harmony House Inc. adding to locations 411 Oxburgh Place Newport News, Va. 104 Hunts Neck Road Poquoson, Va.

  3. The Arkk Group, LLC adding new location for residential group home 915 Mineo Lane Portsmouth, Va. 23701




  1. New Business:

    1. Open Heart added one new location 1100 Merrimac Ave. Norfolk, Va. and closed one location 524 Youngtowns Chesapeake, Va.

    2. Open Hearts Mental Health Skill Building Services was granted affiliation. The program rules of conduct were approved. The program revised behavioral management policies were accepted.

    3. Divine House, Dr. Vimal Amin, will provide clerical support to the LHRC from 04/27/2015- 04/27/2016.



  1. Financial Report: (Provider information only) as of 2/23/2015 there is $700.00 available. There is no activity since last report.



  1. Advocate’s Report:



  1. Mr. Daye would like to thank everyone for submitting their reports on the correct forms. The LHRC has an applicant to interview today to fill the future vacancy when Ms. Copeland finishes her 2nd term on 06/30/2015.

  2. Mr. Daye reminded providers to let the committee know when you adding new home or services. Instead of sending the Human Rights Compliance Verification Form to the Director of the Office of Human Rights in Richmond, send it to Mr. R. Daye.

  3. Please submit, two week prior to next meeting, your 1st quarter reports, covering the period of: Jan.1, 2015-March 1, 2015.

  4. The committee members and Mr. Daye should receive, by mail, the first quarterly report by April 13th 2015 for the April 27, 2015 meeting.

  1. Public Comment Period: No one from the public was present to speak.




  1. Future Meeting Location and Schedule: April 27, 2015, July 22, 2015, and October 26, 2015, all meetings are being held at F.A.C.E.S. Community Services at 3500 Tejo Lane, Suite 105, Chesapeake, VA 23321.



  1. Closed Session: the LHRC motioned, seconded and approved to go into closed session per VA Code 2.2-3711A to conduct a membership interview and to review abuse allegations, neglect and complaint allegations at the following: Arkk Group.

The LHRC voted to come out of closed session.

Upon reconvening into open session each member certified that to the best of their knowledge, only public business matters, lawfully exempted from statutory open meeting requirements and only public matters identified in the motion above to convene the closed session were discussed.



Recommendations:

Membership Interview:

A motion was made, seconded and approved to recommend to the State Human Rights Committee the appointment of Ms. Sabrina Edward to the Atlantic Regional LHRC, effectively 7/1/15.

The Arkk Group: There were no recommendations.

  1. Elections of Officers:

A motion was made, seconded and approved to appoint the following officers:

Ms. Marylin Copeland- Chairperson-from 2/23/15- *6/30/15 *(term end on 6/30/15)

Steven Alexander- Vice-Chair- term: 02/23/15- 06/30/16

Bruce Copeland- term: 02/23/15- 06/30/16
